Description of Change(for a more detailed description see attached proposal):
W.K.Dickson&Company was awarded the design of the Utility Relocations for GDOT SR 4/ 15th Street
from Milledgeville Rd to Government Rd.Due to a drastic change in scope by GDOT the project area while
reduced became more involved.This is because there are several significant conflicts with the storm sewer
system that will need addressing.In addition,to those changes W.K.Dickson is being tasked with filing
necessary permits,schedule of costs,and final plans for relocation and conflict mitigation with the water and
sanitary sewer system(s)within the project area.

TOTAL AMOUNT OF THIS CHANGE ORDER $ 180,000.00
