Action Ministries respectfully submits this amendment request of its Community Development & Block Grant
(CDBG) #16050 seeking a 6 month extension from January 1, 2017 to June 30, 2017. Additional time is being
requested to ensure staff can provide additional services and resources to clients served.
3. For Budget Amendments, provide a Specific and Detailed Explanation for the Budget Amendment.
