This document shall become an amendment to the contract and all provisions of the
contract will apply.
Justification for Change
Brief description of the proposed contract changes and locations:
1. Plumbing Fixture Revisions. During the value engineering pr ocess prior to
contract execution, a cost savings change was proposed to the project Plumbing
Fixture Package. This c hange included pr oviding a diffe rent drinking fountain
and accessories. Upon review of the su bmittal for the proposed change, it wa s
determined the quality did not m eet that of the building. Therefore, the originally
planned dr inking fountain was recomm ended. This change resulted in an
increase to the project of $1,410.27. Refer to Choate Change Order Proposal
#19.
2. Assistant's Office #123, Delete/Add Doors. It was determined that an area of
the main sitting area be conv erted in to an office for the FBO Manager's
Assistance. As a result to the design ch ange, a credit in the amount of $356.32
will be added to the contract. Refer to Choate Change Order Proposal #22.
3. Fireplace Addition. During the value engineerin g process prior to contract
execution, a cost savings change was pr oposed to eliminate a functioning wood
fireplace in the main sitti ng area. However, after ca reful cons ideration it was
determined a fireplac e would be preferred. Therefore, this it em represents
including a propane fireplace to the project. This change resulted in an increase
to the project of $16,031.34. Refer to Choate Change Order Proposal #23.
4. Miscellaneous Electrical Revisions. This item represents modification s
associated with the building electrical, which inc ludes installing new power
circuits and wireless access point data drops per the review of City IT
department. This c hange resulted in an incr ease to the project of $3,623.66.
Refer to Choate Change Order Proposal #24.
5. Site Water Line Revisions. As a result to the rerouting of the site water layout
from the east side of the bui Iding to the west side of the building modifications to
the site water line were required per Augusta Utility Department design
guidelines. The modifications included adding a bac kflow prev enter, PIV and
vault for fire service, and a backflow preventer for domesti c service, and an
additional fire hydrant , gate valv es, tees, fittings, tapping s leeves and v alves.
This change resulted in an increase to the project of $10,556.34. Ref er to
Choate Change Order Proposal #25.
6. Miscellaneous Steel Supports. This item represents modifications associat ed
with the addition of the automatic sliding doors into the vestibule. Work is to
include additional ov erhead supporting st eel for tube steel columns, and a bent
plate needed to carry brick over door opening. Refer to Choate Change Order
Proposal #26.
7. Unfaced Batt Insulation. This item represents a change in the type of insulation
from faced batt insulation to unfaced batt insulation. As a result to this change, a
credit of $950.18 will be added to the contra ct. Refer to Change Order Proposal
#28.
