As a follow-up to your memo, this is to inform you that the property listed above is located in the
Downtown Local Historic Area and the Broad Street National Register Historic District. The requires us to
send Section 106 documentation to the State Historic Preservation office for review, comment(s) and
approval prior to the committin� of anv federal funds.
In order to obtain the preliminary comments of the State Historic Preservation Officer (SHPO), I will
need the following information that applies to the above property:
a) Detailed project description
b) Photographs of structure(s), prints clearly labeled as to property name and indicating location
of all shots
c) Interior photographs of all typical significant and non-significant spaces, labeled as to
description of view and location
d) Photographs of the property's surroundings and adjacent landscape
e) Project location map (copy of GIS map preferred)
f) Site plans
g) Building elevations/lot size
h) Methods and materials proposed for repair or replacement
i) Date of original construction; date(s) of subsequent alterations/additions- brief description of
work and materials used .
j) Alternatives considered priar to demolition of the structure
k) Funding amount used to complete project

The following Tribal Historic Preservation Officer (THPO) would like to be notified regarding the above
property.
a) Notify Eastern Bank of Cherokee Indians (via courtesy email) if disturbing soil classified as
Urban Soil per the most recent Natural resource conservation Service Soil Survey, notify them
via letter if disturbing soil that is not classified as Urban Soil.

b) Consult with the Muscogee (Creek) Nation, OK Creek Nation of Oklahoma for NEW soil
disturbance via emai(only.
c) Seminole Nation of Oklahoma Wewoka Agency would like to be consulted as a consulting
party for projects that require soil disturbance or when a significant cultural resource is
discovered.
d) Seminole Nation Oklahoma (Dania, Big Cypress, and Brighton) would like to review all projects
as THPO, and anytime SHPO is required, this THPO is required. Does not accept emails for
Section 106 Review.

require formaL consultation or mitigation. Complete consultation/mitigation protocol
requirements, publish NOI/RROF and obtain Authority to Use Grant Funds (HUD 7015.16)
