CONTRACT BETWEEN
AUGUSTA, GEORGIA
&
HOPE HOUSE, INC.
EMERGENCY SHELTER GRANT (ESG) PROGRAM
This First Amendment to the Contract executed by and between the Parties on January i, 2oio, is made
and entered into on the � day of December 2oio, by and between AUGUSTA, GEORGIA, (hereinafter
referred to as "GRANTEE"), and HOPE HOUSE, INC., (hereinafter referred to as "SUBRECIPIENT").
WHEREAS, GRANTEE desires to amend the ORIGINAL CONTRAG to allow for additional
funds to help meet the cost(s) of operating the transitional and permanent housing for the treatment of
homeless or extremely low-income women, pregnant women, and women with children suffering with the
disease of addiction and/or disease of addiction with co-occurring mental health illness;
WHEREAS, GRANTEE has determined that this amendment is in its best interest and
SUBRECIPIENT agrees.to said amendment;
NOW THEREFORE, it is mutually understood and agreed by and between the undersigned
contracting parties to amend that previously executed agreement as follows:
(i)
Article II. PROJECT, Paragraph i, is hereby deleted in its entirery and changed to read:
"The Grantee agrees to reimburse the Subrecipient in an amount not to exceed Twenty FiUe Thousand
and One Hundred Tweniy Three Dollars and Twenty Eight cents ($25,i2g.28) hereinafter the "Grant" to
implement the following project(s): "The Comprehensive Mental Health and Substance Abuse Treatment
Program ".

Exhibit A, Section I, SCOPE OF SERVICES Paragraph A. PROGRAM DELIVERY, is hereby deleted in its
entirety and changed to read:
Subrecipient will provide transitional and permanent housing with supportive services for 8 homeless
and extremely low income women. The program is deszgned for women to remain in the program for
i2-24 months. The project operates 24 hours/� days a week at the facility located at 1224 Carrier Place,
Augusta, Georgia. ESG funds will be used towards food, medical/psychological substance abuse
treatment and property insurance for the facility. It is expressly agreed and understood that the total
amount to be paid by the Grantee under this Agreement shall not exceed $2�12�.28. Draw downs for
the payment of eligible expenses shall be made against the line item budgets specified herein and in
accordance with performance.

F.xhibit A, Section II, TIME OF PERFORMANCE, is hereby deleted in its entirety and changed to read:
SerUices of the Subrecipient sha11 start on January 1, 2010 and end on June 30, 2011. The term of
this Agreement and the provzsions herein shall be extended to cover any additional time period
during which the Subrecipient remains in control of ESG funds or other ESG assets.

Exhibit A, Section IV., PAYMENT is hereby deleted in its entirety and changed to read:
It is expressly agreed and understood that the total amonnt to be paid by the Grantee under this
Agreement shall not exceed $25,i2gz8. Draw downs for the payment of eligible expenses shall be
made against the line item budgets specified in Section III herein and in accordance with
performance. Expenses for general administration shall be paid against the line item budgets
specified in Section III and in accordance with performance.
Payments may be contingent upon certification of the Subrecipient's financial management system in
accordance with the standards specified in 24 CFR 84.2i.
Subrecipient is also responsible for submitting to the Grantee, monthly progress reports, time sheets
(if applicable), mileage (if applicable), invoices and any other documentation deemed necessary by
the monitoring official during the funding cycle. These records shall be retained up to five (5) years
after the Agreement expires.
Any amendments to the budget must be in writing and approved by the Grantee's Director of Housing
and Community Development Departmenf.
All other pro�risions of CONTRACT between said PARTIES shall remain in full force and effect.
