CONTRACT BETWEEN
AUGUSTA, GEORGIA
CSRA ECONOMIC OPPORTUNITY AUTHORITY, INC.
Housing Counseling Services
This Fourth Amendment to the Contract executed by and between the Parties on January 13, 2009, is
made and entered into on the thirtieth day of December 2010, by and between AUGUSTA, GEORGIA,
(hereinafter referred to as "GRANTEE "), and CSRA ECONOMIC OPPORTUNITY AUTHORITY,
INC., (hereinafter referred to as "CONSULTANT ").
WHEREAS, GRANTEE has amended the ORIGINAL CONTRACT thus far as follows:
Amendment #1, dated May 19, 2009 — ORIGINAL CONTRACT amount increased from $250,000 to
$285,000 and budget revised to reflect an addition of $13,300 for HOMEFIRST salary and an addition of
$21,700 for HOMEFIRST marketing;
Amendment #2, dated July 1, 2009 — amendment #1 to ORIGINAL CONTRACT is revised transferring
$13,300 in salary funds from HOMEFIRST to CONSULTANT's budget and transferring $21,700 for
marketing from HOMEFIRST to CONSULTANT's budget;
Amendment #f dated April 13, 2010 extends the ORIGINAL CONTRACT expiration date from April
30, 2010 to December 31, 2010 and adds an additional $125,000 to said contract;
WHEREAS, GRANTEE desires to amend the ORIGINAL CONTRACT for the fourth time to extend the
contract expiration date, change the scope of services and revise the budget as set forth in amendment #3,
dated April 13, 2010;
WHEREAS, GRANTEE has determined that this amendment is in its best interest and
CONSULTANT agrees to said amendment;
NOW THEREFORE, it is mutually understood and agreed by and between the undersigned
contracting parties to amend the aforementioned executed agreement, as follows:

WITNESSETH, Paragraph 2, is hereby deleted in its entirety and changed to read:
"WHEREAS, the Grantee desires to engage the Consultant for the purpose of providing Housing
Counseling services to low and moderate income households."

EMPLOYMENT OF CONSULTANT, Paragraph #i is hereby deleted in its entirety and
changed to read:
"EMPLOYMENT OF CONSULTANT. GRANTEE agrees to engage the Consultant and the Consultant
agrees to provide Housing Counseling services."

Appendix A, Scope of Services, is hereby deleted in its entirety and changed to read:
APPENDIX A — SCOPE OF SERVICES
I. SCOPE OF SERVICES
A. Activity — Housing Counseling
The Consultant will provide to low and moderate income persons Housing Counseling services to first -
time homebuyers to include, but not limited to:
1. Outreach
2. Information on Homebuyer Financial Assistance Programs
3. Group Homebuyer Education Seminars
4. One -on -One Pre - Purchase Counseling
5. Post - purchase Counseling
6. Default /Delinquency /Foreclosure Intervention Counseling
It is anticipated that approximately 900 households will be provided the aforementioned services with
51% being low to moderate income households for the term of the contract (April 2009 through June 3o,
2011).
B. Special Events — Consultant will collaborate with the Grantee in sponsoring the
following events:
1. Fair Housing Fair in April.
2. Homeownership Fair in June

*Initially, Consultant proposed that Housing Counseling would begin with a one -hour group orientation
held monthly. The purpose of the orientations would be to familiarize clients to homeownership services
and staff, assess readiness to purchase, and determine the next step for individual housing counseling.
Upon contracting with HomeFirst Housing Resources Services, Inc. their model of counseling used a
monthly group Homebuyer Education Class as the point of entry for clients, thus this replaced monthly
orientations.
C. General Administration - Staffing
1. Administrative Housing Counselor — Counseling emphasis on long -term pre-
purchase and foreclosure intervention counseling
2. Mortgage Counselor and Financial Advisor — Counseling emphasis on foreclosure
intervention counseling
3. Housing and Marketing Coordinator - Counseling emphasis on short-term
pre - purchase and foreclosure intervention counseling
4. Administrative Assistant — Provides administrative support to
counselors that includes scheduling appointments and assisting with client
intake and follow up
II. Time of Performance
Services of the Consultant shall start on January 13, 2009 and end June 3o, 2011 (based on
budget, this funding is projected to be expended by March 31, 2011). The term of this Agreement and the
provisions herein shall be extended to cover any additional time period during which the Consultant
remains in control of grant funds or other assets, including program income.

Compensation requests for payment may be submitted monthly with a 30 -day turnaround for payments.
Total payments for the services rendered under this Contract will not exceed $410,000.

The following documentation must accompany payment requests for budget line items above:
For Salary — Time Sheets must be submitted indicating daily work hours (time -in, time -out) and daily
work tasks. Hours devoted to CDBG activities must be separate from other work hours. Payroll register
and cancelled check must be submitted also.
Rent — Invoice, cancelled check and copy of lease, if not already on file.
For Office, Program Supplies & Equipment — Three (3) quotes must be obtained for all purchases. Quotes
must be recorded on "Quote Form ", invoice and cancelled check.
For Travel — Travel Log must be maintained and submitted with reimbursements. Mileage will be paid at
the rate of .5o per mile. For out -of -town travel, consultant must obtain approval from Grantee.
For Training — Conference registration fee itemization, Conference agenda, hotel bill and cancelled check.
Telephone and Internet Service — Copy of actual bill and cancelled check.
Postage — Copy of invoice /receipt and cancelled check.
Computer Service — Copy of invoice and cancelled check.
Direct Services — Copy of invoice /receipt and cancelled check.
Administration - Liability insurance cancelled check.

Appendix C, Project Monitoring, is hereby deleted in its entirety and changed to read:
APPENDIX C, REPORTING
A. Performance Monitoring
The Consultant agrees that the Grantee may carry out periodic monitoring of activities as deemed
necessary. At a minimum, monitoring shall occur annually to also to include 2 sites visits during the
program years. Grantee will provide the Consultant with advance notice in writing prior to any
monitoring. Such monitoring shall consist of evaluating the Consultant's compliance with the terms and
conditions of this Agreement, project progress and performance.
B. Monthly Reports
The Consultant shall submit monthly reports to Grantee by the 15th day of the month for prior month
activities.
The Grantee will monitor the performance of the Consultant against goals and performance standards as
stated in Appendix A. Substandard performance as determined by the Grantee will constitute
noncompliance with this Agreement. If action to correct such substandard performance is not taken by
the Consultant within the time designated by Grantee, contract suspension or termination procedures will
be initiated.
C. Annual Performance Report
The Consultant will be responsible for providing an Annual Performance Report that includes a detailed
summation of accomplishments. The Annual Performance Report is due January 15 for prior activities.
D. Report Schedule
