RESOLUTION TO AUTHORIZE EXECUTION OF ASSET TRANSFER FORM FOR
ACCOUNTS SUBJECT TO THE 1998 AUGUSTA MONEY PURCHASE PENSION
PLAN
WHEREAS, the Augusta-Richmond County Commission, as the governing authority of
Augusta, Georgia, hereinafter referred to as the "Employer" wishes to authorize the Mayor or
Mayor Pro Tem to execute an amended asset transfer form for use in liquidating the accounts of
its employees at Nationwide and transfer those accounts to the GMA 2008 Defined Contribution
Plan or the 2008 GMA Defined Benefit Plan as authorized by each Employee election, it is
hereby
RESOLVED, that the amended transfer form presented at this meeting as finally
approved by legal counsel is hereby approved and adopted and that the duly authorized agents of
the Employer are hereby authorized and directed to execute and deliver to the Administrator of
the Plan one or more counterparts of said form.
