Dear Mr, Cantu,
Emergency Reporting is pleased to offer the City of Augusta an additional software
agreement, based on our conversation to this point.
Attached to this document you will find "Attachment An that outlines the terms and
conditions of the agreement. Due to the highly competitive nature of our industry, we
request that all documents and correspondence with Emergency Reporting be
considered confidential and restricted to authorized parties representing the City of
Augusta.
The terms and conditions of the previous software agreements will continue to apply.

Attachment A
Monthly Service Description
1. Emergency Reporting will provide server hosting for the City of Augusta Fire
Department (AFD) located in Bellingham, WA. AFD personnel are to enter in
response information to the website located at www.emergencyreporting.com.
2, Once the servers are purchased, installed and operating in the City of Augusta's
network, Emergency Reporting will transfer all agency information to the City of
Augusta servers. Emergency Reporting will have up to fifteen (15) calendar days
to perform the transfer. A longer period of time is allowable with the joint
agreement of the City of Augusta IT representative and the Emergency Reporting
representative.
- ,-,- --3. There-will-be-a-period of-time:ran-ging in-length-fr6m approximately--8-to -36 nours,
when the system will not be available on either the Emergency Reporting servers
or the City of Augusta servers. This time is required to facilitate transfer of the
data between the two systems.
4. The monthly fee shall be $129 per station, per month. For the terms of this
agreement, the City of Augusta will have twenty (20) stations. The monthly
service fee is $2,580 (129 * 20).
5. The initial agreement begins service on December 6th, 2004, and ends service on
February 6th, 2005. Each additional 30-day period of service will be charged at
the same rate as item four.
6. This agreement expires on July 6,2005.
