Georgia 30906 I-lAND DELIVERY Re: Kissingbower Road Area Sanitary Sewer Extension, Phases I V and V Our File No. 96-088.0 I Dear Robin: In accordance with your request. we are pleased to offer the following proposal for revising our current construction plans, providing construction phase services. and for providing as-built record drawings for the sanitary sewer project located on and near Kissingbower Road. George Road. Haynie Drive, and Milton Road. I. REVISIONS TO CURRENT CONSTRUCTION PLANS According to the review of the current plans by CH2M Hill and our subsequent meeting on Thursday. September 14. 2000. we have listed below the revisions that were requested. A, We propose to: I, Update existing plan to reflect the current government otticials in office, 2, Revise the name on each sheet from Augusta-Richmond County Utilities to Augusta Uti I i ties Department. 3. Provide X.y.z val LIes for each manhole, including I'inished 1 id elevation with a qual ifying note as to the expected tolerance. Coordinates based on assumed system with control monuments shown on plans. 4, Add stationing for each match line shown. 5. Move sewer taps out of manholes. 6, Acid note specifying type of sewer piping. 7, Change detail of "Trenching, Backfilling and Repaving" to match current AugLlsta- Richmond County Public Works Department requirements./ D b.~'>i" L/: .. . ;; ~ 1\1s, Robin McMillon September 22. 2000 Page 2 B, We "viii revise the contract documents per CH2M Hill's technical review memo to: I, Reflect current government officials. 2. Meet current Augusta Utilities Department standards. ], Add supplementary condition adding program manager. 4, Provide contract duration in proposal and agreement sections. 5. Revise asphalt overlay dimension, 6. Move additional items at unit prices to contract items with an estimated quantity and add riprap to list. 7, Revise technical specifications for excavating, trenching and bacldilling for pipe lines to match the current Augusta-Richmond County Public Works Department standard. i{, Correct technical specification labeling of the Measurement and Payment Section hom -06 to -07. <), Revise technical specitication for ductile iron pipe 10 specify epoxy lining in accordance with the standard specifications of the Augusta Utilities Department. Fee Proposal Our tee for this work will be a lump sum of$I,640.00, Time of Completion We are prepared to begin work at your direction and expect to complete the job within two weeks of receiving your notice to proceed. II. CONSTRlJCTION PHASE SERVICES We propose (0 furnish the following construction phases services as required: assisting you with securing bids li'om qualitied contractors, advising you concerning the award of a contract, issuing a notice to proceed, making periodic observations of the construction work as it progresses to observe the progress and general quality, furnishing wri tten observation reports of the observations made during each visit, issuing instructions to the contractor, reviewing the contractor's monthly applications for partial payment, determining the amounts owing to the contractor, i nterpreti ng the con tract documents and technical speci fications. consulti ng as necessary wi th Augusta Utilities Department and CH2M Hill program manager, making a linal inspection to determine whether or not the work has been completed substantially in accordance with the plans and specilications. and reviewing the contractor's application for tinal payment. Any observation services provided will be for the purpose of monitoring compliance with the plans. specifications. and contract documents, and will not in any manner be a guarantee of the methods, appliances. or schedules employed by the contr8ctor, nor for the safety of the job, i. ~ . ~ . Ms, Robin McMillon September 22, 2000 Page 3 Fee Proposal Our fee for this work will be on a time and material basis not to exceed $5,760.00, according to our rates in effect at the time services are provided. A current rate table is attached. This fee is based on the construction contract duration of 180 days. For any increase in the length of the contract, we would expect to provide additional services on a time and material basis, III. RECORD DRA W.lNGS We propose to furnish final as-built drawings showing northern and easting values (Georgia State Plane Coordinates) for each sanitary manhole and to provide elevations for manhole lids and inverts, Sewer service locations will be shown using the dimensions from the field prints provided by the contractor. This information will be shown on the construction plans and will be provided to Augusta Utilities Department on mylar and electronically in CAD f(wmat. Fee Proposal Our fee for this work will be a lump slim of $5,1 60.00.
