RJL- Time Frame: Complete through Design Phase 45 days from Notice to Proceed Amount Proposed: Scope Items I, III thru V: $36,466 Lump Sum Scope Item II: Varies from $3,200 to $5,800 (Note: The minimum amount is contingent on encountering subsurface conditions favorable to the economical design of shallow footings for a composite or steel tank. The maximum amount includes investigations and recommendations for design of deep foundations-piles, caisson foundations, etc., including drilling 10 feet into sound rock.) Particulars: 1. This fee proposal is presented on behalf of the Augusta Design Group (ADG). Work will be I prepared by JLA doing business as ADG. 2. See attached Scope of Services (2 Pages). 3. See attached Fee Proposal Worksheets (3 Pages). 4. Since the cost for geotechnical services is presented as a variable, the invoiced amount submitted to ADG will be submitted as back-up information to ADG's invoices for payment. Upon acceptance of this Proposal Johnson, Laschober & Assoc., p.e will prepare a Contract with. Terms and Conditions for signature. (J,UJ\) cr File: H:IPROJMAMPROPOSALIPZL 1997\PZL 139,WPD ... .' -. .. SCOPE OF SERVICES FOR ONE MILLION GALLON (1 MG) HIGHPOINTE ELEVATED STORAGE TANK I. Topographic Survey (Task 3) A. Establish USGS Datum at proposed Highpointe tank site. B. Perform topographic survey at proposed Highpointe tank site - including access road and tie into Frontage Road. C. Confirm high water level elevation 564.25 on existing 0.5 MG tank at Highland Avenue. II. Subsurface Investigation and Report (Task 3) A. A minimum offour(4) borings will be provided. Three (3) borings will be equally spaced on a minimum circle equal to the tank diameter with the tank as the center. One,(1) boring shall be located at the tank center. B. Tests shall include Standard Penetration Tests, Soil Classification and measurement of water table. C. Provide report with findings and recommendations for foundations for composite tank with a single concrete support shell and for steel tank with multiple leg supports and center riser. III. Elevated Storage Tank - Conceptual thru Design Phase (Tasks 2,3, & 4) A. Establish tank parameters with regard to vertical operating range. Confirm through Augusta-Richmond Utilities Department tank design fill and withdrawal rate parameters. (Note: 'Tank high water level will be controlled 1) by the existing pressure reducing valve lo~ted on the Frontage Road (separating the 630 and 564 levels) and 2) by tying into the SCADA system (providing control between pumps at Highland Ave. and the tank).) 8 Define tank accessories. C. Investigate steel and composite tank designs and provide a comparative analysis. Evaluate available head ranges for each tank type with respect to project requirements. Prepare contract documents including: 1. Technical Specifications a. Earthwork b. Foundation systems c. Composite elevated storage tank or steel elevated storage ta~k or both. d. Valves and piping e. Fence f. Miscellaneous sitework 2. Elevated storage tank arrangement type drawings, Le. elevations, plans, details, etc. Recommended foundation systems will be provided for each tank type. 3. Site plans with location and layout, grading and drainage, soil erosion and sediment control plan information. Plan will also include driveway entrance detail, vicinity map and details. 4. Submit plans for agency approval as required. IV. Elevated Storage Tank - Bid and Contract Phase (Task 5) A. Prepare advertisement for bid and prepare plans and specifications for ,distribution to contractors. B. Pre-Bid Conference and Addenda. C. Assist Augusta-Richmond Utilities Department during bid opening, evaluation of bids and recommendation of lowest qualified bidder. D. Send out Notice of Award upon engineering service and commission approval for award and assist in obtaining signed contracts with insurance certificate and bonds. E. Review shop drawings, design calculations and material submittals. C. Respond to requests for information and provide technical support to Augusta- Richmond Utilities Department as required. (Note: Daily inspection, as required, will be provided by Augusta-Richmond Utilities Department personnel.) JLA will monitor the construction activity on a monthly basis. D.
