reporting forms (for both financial and progress reports) with
instructions for preparation of the forms. All copies of the
progress and financial reports, as required under the terms of
your Award, should be sent to FMD, which will subsequently
forward them to your Program Manager. Progress reports must be
submitted within 30 days after the end of the reporting periods,
which are June 30 and December 31 for the life of the Award. The
awarding agency may opt, by special condition to the Award, to
combine the first report into the subsequent reporting period.
For example, if the begin date on the Award is June 1st, the
awarding agency may opt to receive the first report 30 days after
the December 31st reporting period.
By accepting this Award, you assume certain administrative
and financial responsibilities including the timely submission of
all financial and programmatic reports, the resolution of all
interim audit findings, and the maintenance of a minimum level of
grant cash-on-hand. Should your organization not adhere to the
terms and conditions of this Award, it is subject to termination
for cause or other administrative action, as appropriate.

Congratulations on your recent award. Because you have submitted Certified Assurances that your agency is
in compliance with applicable civil rights laws, this office has determined that you have met this requirement
in the Department of Justice regulations governing recipients of Federal financial assistance (see 28 C.F.R. ~
42.204, Applicants' Obligations). As Director ofthe Office for Civil Rights (OCR), Office of Justice Programs,
I would like to offer you my assistance in completing the conditions of these Assurances, specifically Nos. 13,
14, and 15, as the grant goes forward.
As you know, equal opportunity for the participation of women and minority individuals in employment and
services provided under programs and activities receiving Federal financial assistance is required by law.
Therefore, if there has been a federal or state court or administrative agency finding of discrimination against
your agency, please forward a copy of such order or consent decree, as required by Assurance No. 14, to OCR

Additional Instructions For Grantees ~eceivinl! $500.000 Or More:
1. In accordance with Assurance No. 15, ~ach grantee that receives $500,000 or more (or $1,000,000 in
an 18- month period), and has 50 or more employees, must submit an Equal Employment Opportunity
Plan (EEOP)within 60 days from the date of this letter to OCR at the above address. 3
2. Alternatively, the grantee may choose to complete an EEOP Short Form, in lieu of sending its own
comprehensive EEOP, and return it to OCR within 60 days of the date of this letter. This easy-to-
follow EEOP Short Form reduces paperwork and preparation time considerably and will ensure a
quicker OCR review and approval. The enclosed Seven-Step Guide to the Design and Development
of an EEOP (which includes an EEOP Short Form) will assist you in completing this requirement.
3. Please be reminded that the above requirements apply to primary grantees and to each of their
sub grantees or contractors that meet the criteria outlined in this letter. Therefore, all primary grantees
should apprise sub grantees of these responsibilities and those meeting the criteria should send their
EEOPs or EEOP Short Forms directly to the Office for Civil Rights within 60 days of the date of their
award.
3If you have already submitted an EEOP as part of another award from the Office of Justice Programs
(OJP) or the Office of Community Oriented Policing Services (COPS) within this grant period, or if you have
certified that no EEOP is required, it is not necessary for you to submit another at this time. Simply send a
copy of the letter you received from OCR showing that your EEOP or certification is acceptable.
NOTE: If agency has under 50 employees, regardless of amount of award, no EEOP is required; however '
grantee must r~rn applicable portion of certification Fonn to OCR within 60 days., .

SPECIAL CONDITIONS
1. The recipient agrees to comply with the financial and
administrative requirements set forth in the current edition
of the Office of.Justice Programs (OJP) Financial Guide.
2. The recipient agrees to comply with the audit requirements
of OMB Circular A-133:
The first audit should cover the period 10/1/97 through
Ti-li(/98. Include the audit report and accompanying
management letter,in the audit report package.
If the audit disclosed findings or recommendations, the
recipient must also include a corrective action plan, in the
audit report package, that contains the following:
1) The name and number of the contact person responsible for
the corrective .action plan.
2) Specific steps taken to comply with the recommendations.
3) Timetable for performance and/or implementation dates for
each recommendation.
4) Descriptions of the monitoring to be conducted to ensure
implementation.
Submit audit report packages and the appropriate number of
copies as defined inOMB Circular A-133 & .320, no later
than thirteen (13) :months after the close of each fiscal
year during the term of the award (for fiscal years'
beginning on/after 1 July 1998 audit reports packages are
due nine (9) months after the close of the 'fiscal year) as
follows:

3. Approval of this grant does not indicate an approval of any
consultant rate in excess of $450 per day. Specific and
detailed justification must be submitted to and approved by
the Executive Office for Weed and Seed prior to obligation
or expenditure of such funds.
4. Budget item submissions for equipment and other contract
items are accepted as best estimate on~y and are not deemed
approved at that price. Recipients are required to ensure
such items are not currently available, are not duplicative
or excessive, and should make market surveys and obtain the
best prices available.
5. Grantee acknowledges that failure to submit an acceptable
Equal Employment Opportunity Plan (if grantee is required to
submit one pursuant to 28 CFR Section 42.302), that is
approved by the Of~i~e of Civil Rights, is a,violation of
its Certified Assurances and may result in the suspension of.
the drawdown of funds.
6. Grantee agrees to require that organizations which receive
grant funds certify, as part of the contract, that they have
appropriate hiring policies and screening procedures for
employees who will be working with youth and other residents
as part of the'Weed and Seed strategy.
7. The grantee agrees to conduct at least one Safe ~aven'
Program as part of their overall Weed and Seed strategy.
8. While specific program strategies may vary from site to
site, the planning, development and execution for the
following elements shall be common to'all Weed and Seed
sites. These common elements are: (1) to organize and
regularly convene a Weed and Seed steering committee; (2) to
maintain focus on the four components in the target
neighborhood; (3) to screen applicants working with children
while selecting and mobilizing resources to'address
neighborhood problems; (4) to 'regularly revisit goals,
objectives, and the implementation strategies and schedules.
Failure to substantially comply with these elements are
cause to discontinue grant funding.

The Augusta Weed and Seed Program Funding Application includes the four integral components of the national Weed
and Seed program: law enforcement; community policing; prevention/intervention/treatment; and neighborhood
restoration. All program funding and activities will directly ,benefit the Barton Village neighborhood, located in
the southwest part of the city of Augusta.

This project site will direct a,comprehensive, multi-agency program entitled, "Weed and Seed" to coordinate the
delivery of criminal justice and social services to eliminate violent crime, drug-trafficking, and drug-related
crime and to provide a safe environment for law-abiding citizens to live, work, and raise a family. Through ~he
application of coordinated law enforcement and criminal justice services,in a targeted neighborhood, this program
will nweec1~ from that neighborhood criminal offenders engaged in drug crimes and other violent offenses, stabilize
the neighborhood through community-oriented policing, and "seed" that neighborhood with housing, employment and
