Homeless Objective #1 (HOM1) - Provide case management services to the homeless and near-homeless in order to assess needs and make appropriate referrals. Goal: Case management is a top priority in the CP and the City expects to provide 2,500 homeless/near-homeless individuals/families from 2005- 2009. The HPRP funds will allow the City to provide more assistance to organizations so that case management is provided by the agencies. . Homeless Objective #2 (HOM2) - Increase the number of housing units available to the homeless. Goal: The CP goals are to provide 50 units of permanent housing and 50 units of permanent supportive housing units. With HPRP funds, organizations will have more funds available that will enable them to provide financial assistance and stabilization services to prevent homelessness and rapidly re-house individuals and families. Therefore, the supply of permanent and permanent supportive housing units will increase. . Homeless Objective #3 (HOM3) - Increase the chances of the homeless returning. to self-sufficiency by providing access to supportive services, including the expansion of existing services or the development of new services that meet their most critical needs. Goal: The City's goal in the CP is to provide additional ESG and CDBG funding to agencies and organizations serving the homeless, near homeless and those with special needs. HPRP funds are an additional funding source that the City is awarding to organizations. This will allow organizations to serve more clients and prevent homelessness and stabilize households.
