Substantial Amendment to the Consolidated Plan 2008 Action Plan for the
Homelessness Prevention and Rapid Re-Housing Program (HPRP)

Grantees eligible to receive funds under the Homelessness Prevention and Rapid Re-
Housing Program (HPRP) are required to complete a substantial amendment to their
Consolidated Plan 2008 Action Plan. This form sets forth the required format for this
substantial amendment. A completed form is due to HUD within 60 days of the
publication of the HUD HPRP notice.

To aid grantees in meeting this submission deadline, the HPRP Notice reduces the
requirement for a 30-day public comment period to no less than 12 calendar days for this
substantial amendment. With this exception, HPRP grantees are required to follow their
Consolidated Plan's citizen participation process, including consultation with the
Continuum of Care (CoC) in the appropriate jurisdiction(s). Grantees are also required to
coordinate HPRP activities with the Coe's strategies for homeless prevention and ending
homelessness. To maximize transparency, HUD strongly recommends that each grantee
post its substantial amendment materials on the grantee's official website as the materials
are developed.
A complete submission contains the following three documents:
1) A signed and dated SF-424,
2) A completed form HUD-40119 (this form), and
3) Signed and dated General Consolidated Plan and HPRP certifications.
For additional information regarding the HPRP program, visit the HUD Homelessness
Resource Exchange (www.hudhre.info).This site will be regularly updated to include
HPRP resources developed by HUD and its technical assistance providers.
The information collection requirements contained in this application have been submitted to the Office of
Management and Budget (OMB) for review under the Paperwork Reduction Act of 1995 (44 V.S.c. 3501-
3520). This agency may not collect this information, and you are not required to complete this form, unless
it displays a currently valid OMB control number.
Information is submitted in accordance with the regulatory authority contained in each program rule. The
information will be used to rate applications, determine eligibility, and establish grant amounts.
Public reporting burden for this collection of information is estimated to be 16 hours, including the time for
reviewing instructions, searching existing data sources, gathering and maintaining the data needed, and
completing and reviewing the collection of information. This information is required to obtain benefits. To
the extent that any information collected is of a confidential nature, there will be compliance with Privacy
Act requirements. However, the substantial amendment to the Consolidated Plan 2008 Action Plan does not
request the submission of such information.

B. Citizen Participation and Public Comment
1. Briefly describe how the grantee followed its citizen participation plan regarding
this proposed substantial amendment (limit 250 words).
Response:
After Congress publication of the Homelessness Prevention and Rapid Re-Housing
Program (HPRP) notice on March 19, 2009, the City held a meeting with the
Continuum of Care members to explain the details of the program. At this
meeting, the City invited the members to attend the upcoming public hearing.
In accordance with the City of Augusta's Citizen Participation Plan, a public notice
was published in the Augusta Chronicle March 27, 2009, announcing a substantial
amendment to the Consolidated Plan's 2008 Action Plan. The purpose of the
amendment is to include the Homelessness Prevention and Rapid Re-Housing
Program (HPRP) in the 2008 Action Plan and the funds allocated to the City, which
is $927,319.
In the same public notice, the City announced the March 30, 2009, public hearing
and the commencement of the application process. The deadline for submission of
proposals was April 6, 2009.
The proposed projects, to be included in the "substantial amendment" to the 2008
Annual Action Plan were published in the Augusta Chronicle on April 13, 2009,
the Metro Courier on April 14, 2009 and posted on the City's Web site on April 13,
2009. The 12-day public comment period on the proposed projects was April 13,
2009 through April 24, 2009. As a result of the publication, no comments were
received from citizens or any other interested parties.
On May 5, 2009, the Augusta Commission approved the "substantial amendment"
and authorized submission of the application for HPRP funds to HUD.
On April 29, 2009, a Public Notice was published in the Augusta Chronicle and the
Metro Courier announcing to the public that HUD-40 119 "Substantial Amendment
to the Consolidated Plan 2008 Annual Action Plan for the Homelessness
Prevention and Rapid Re-Housing Program" was available for review and
comments. The 12-day comment period started April 29, 2009 and expired May
11,2009. Additionally, the public was notified that a copy of the amendment was
available at the Augusta Housing and Community Development Department, ARC
Planning Commission and the Mayor's office for review. In addition, the
"substantial amendment" was posted on the City's Web site on April 29, 2009.
The City did not receive any comments.

C. Distribution and Administration of Funds
Reminder: The HPRP grant will be made by means of a grant agreement executed by HUD
and the grantee. The three-year deadline to expend funds begins when HUD signs the grant
agreement. Grantees should ensure that sufficient planning is in place to begin to expend
funds shortly after grant agreement.

2. Briefly describe the process(es) indicated in question 1 above (limit 250 words).
Response:
The City utilized the competitive Request for Proposal (RFP) process to solicit
sub grantees for the homeless prevention, rapid re-housing and housing stabilization
activities. As indicated under paragraph "B - Citizen Participation and Public
Comment" above, the deadline for proposals was April 6, 2009. The City received
fourteen (14) proposals from non-profit organizations. Proposals were evaluated by
the Augusta Housing and Community Development staff according to the criteria
set forth in the RFP.
The City is committed to administering the HPRP funds in compliance with the
HPRP regulations and implementing the projects in a timely manner. Recognizing
that effective implementation of the HPRP will depend on the ability to have a
workable delivery system in place, the City selected qualified organizations with
experience providing services to the homeless and administering homelessness
prevention and rapid re-housing programs. These organizations are competent and
has the ability to provide prompt services that will be beneficial to the homeless
and persons at-risk of being homeless.
Of the fourteen (14) proposals received, the following ten (10) organizations were
recommended for funding:

Of the ten (10) agencies selected, only one sub grantee is not a member of the
Augusta Continuum of Care Committee, although this organization has experience
working with the homeless and homeless programs. The City will make it
mandatory that this sub grantee become a member of the CoCO

Additionally, nine (9) of the above sub grantees are already users of the Homeless
Management Information System (HMIS). The tenth organization will become a
user of HMIS immediately upon release of HPRP funds.
3. Briefly describe the process the grantee plans to use, once HUD signs the grant
agreement, to allocate funds available to subgrantees by September 30, 2009, as
required by the HPRP Notice (limit 250 words).
Response:
Once the City receives HUD's grant agreement, the grant agreement will be
presented to the Commission for acceptance of the funds on behalf of the City of
Augusta. The City will timely return the executed grant agreement within the 15-
day time period toHUD.
As indicated in paragraph C.2 above, the subgrantees for funding have been
selected. City staff plans to have the grant agreements for the sub grantees
completed by July 2, 2009, and send award letters along with the agreements out to
the subgrantees for execution in an effort to have the agreements fully executed by
the end of August 2009.
Beginning October 1,2009, subgrantees will begin providing assistance to clients.
4. Describe the grantee's plan for ensuring the effective and timely use ofHPRP grant
funds on eligible activities, as outlined in the HPRP Notice. Include a description
of how the grantee plans to oversee and monitor the administration and use of its
own HPRP funds, as well as those used by its sub grantees (limit 500 words).
Response:
Program planning has begun and will continue through August 2009, so that
implementation of the activities may begin on October 1, 2009. Planning include
development of forms, sub grantee agreements, development of a HPRP Policies
and Procedures Manual and training of sub grantees.
Subgrantee agreements will cover a 30-month time period from October 1, 2009
through March 31, 2012. The agreements will have timeline benchmarks for
expenditure of funds based on 6-month intervals at a minimum expenditure rate of
20% per interval.
Subgrantees will be required to submit mandatory monthly progress reports, HMIS
reports, annual reports and attend monthly partner (CoC) progress meetings.
The City will use its current subrecipient monitoring procedures as a basis to ensure
. timely expenditures of the grant funds. .The City will conduct quarterly and annual

Substantial Amendment to the Consolidated Plan 2008 Action Plan for the
Homelessness Prevention and Rapid Re-Housing Program (HPRP)
on-site grant monitoring visits to ensure compliance with grant agreement and to
ensure that expenditures are on schedule.
The Grantee's lead persons will be the Assistant Director-Programs and the
Community Development Manager. Four (4) Community Development
Coordinators will provide oversight of assigned agencies. The CD Manager will
ensure timely submission of subgrantee monthly, quarterly and annual reports. The
HMIS Coordinator will monitor and evaluate HMIS data. The Assistant Director-
Programs will monitor compliance with the four eligible categories; financial
assistance, housing relocation and stabilization services, data collection and
evaluation, to ensure that funds are being expended in a timely manner, and closely
monitor HUD timelines to meet the required reporting requirements.
City staff will meet on a monthly basis to monitor administration of the funds,
program implementation, program performance and compliance with federal
requirements.
It is the City's plan and intent to expend 60% of the funds within the required two
(2) years and 100% of the funds within the required three (3) years. The City will
maintain monthly expenditure spreadsheets to track expenditures for adherence to
benchmarks.
D. Collaboration
1. Briefly describe how the grantee plans to collaborate with the local agencies that
can serve similar target populations, which received funds under the American
Recovery and Reinvestment Act of 2009 from other Federal agencies, including the
U.S. Departments of Education, Health and Human Services, Homeland Security,
and Labor (limit 250 words).
Response:
City staff has already started collaborating with local agencies that. may receive
American Recovery and Reinvestment Act (ARRA) funds in order to set the venue
for future communications that will help identify how HPRP funds might be
coordinated with other services. Collaboration will take place through meetings,
telephone conversations and written communications. Coordination with other
services funded under the ARRA may include: child care, healthcare, TANF and
other public assistance, employment services, unemployment benefits, tax credits,
education, domestic violence services, FEMA, low-income housing and other
servIces.

2. Briefly describe how the grantee plans to collaborate with appropriate
Continuum(s) of Care and mainstream resources regarding HPRP activities (limit
250 words).
Response:
The City of Augusta's CoC currently maintains a close relationship with local
mainstream resources. The City's CoC members consist of representatives from
the Department of Labor, Richmond-Burke Job Training Authority, Department of
Family and Children Services (DFCS), Department of Veterans Affairs Medical
Center, Serenity Behavioral Health Center and the Richmond County Board of
Education. The City's CoC is also fostering relationships with representatives from
the Social Security Administration and Homeland Security. These entities will keep
the CoC membership informed of their homeless activities and funding
opportunities.
The Richmond County Board of Education ARRA grant will provide: After-school
tutorial assistance, Pre-school and summer programs, before and after-school care,
special dues and fees, school supplies, case management services and
transportation to medical appointments for homeless children.
oUr partners at the Department of Veterans Affairs, recipients' of HUD V ASH
program has partnered with the local Public Housing Authority to provide Section
8 housing vouchers which has streamlined the process for their homeless veterans.
A process that once could have taken months, or even years, now is a three to four
week period. The V A's V ASH Coordinator provides intense case management for
these veterans. The V A V ASH Coordinator will partner with other CoC partners to
provide a seamless continuum of serves for these veterans.
The City of Augusta and its CoC partners are actively collaborating and applying
for various grants that all tie to our Consolidated Plan, the CoC and our 1 0- Year
Plan to End Homelessness.
3. Briefly describe how HPRP grant funds for financial assistance and housing
relocation/stabilization services will be used in a manner that is consistent with the
grantee's Consolidated Plan (limit 250 words).
Response:
The City's 2005-2009 Consolidated Plan identifies strategies for assisting persons
who are homeless or at risk of homelessness to met their needs for housing stability
through prevention and re-housing strategies. The HPRP funds for financial
assistance and stabilization services will be utilized to target families at risk of
homelessness and those who have recently become homeless in a manner that is
consistent with the Consolidated Plan (CP) goals which are listed below:

. Homeless Objective #1 (HOM1) - Provide case management services to the
homeless and near-homeless in order to assess needs and make appropriate
referrals.
Goal: Case management is a top priority in the CP and the City expects to
provide 2,500 homeless/near-homeless individuals/families from 2005-
2009.
The HPRP funds will allow the City to provide more assistance to
organizations so that case management is provided by the agencies.
. Homeless Objective #2 (HOM2) - Increase the number of housing units
available to the homeless.
Goal: The CP goals are to provide 50 units of permanent housing and 50
units of permanent supportive housing units.
With HPRP funds, organizations will have more funds available that will
enable them to provide financial assistance and stabilization services to
prevent homelessness and rapidly re-house individuals and families.
Therefore, the supply of permanent and permanent supportive housing units
will increase.
. Homeless Objective #3 (HOM3) - Increase the chances of the homeless
returning. to self-sufficiency by providing access to supportive services,
including the expansion of existing services or the development of new
services that meet their most critical needs.
Goal: The City's goal in the CP is to provide additional ESG and CDBG
funding to agencies and organizations serving the homeless, near homeless
and those with special needs.
HPRP funds are an additional funding source that the City is awarding to
organizations. This will allow organizations to serve more clients and
prevent homelessness and stabilize households.

GENERAL CERTIFICATIONS FOR STATE OR LOCAL GOVERNMENT FOR THE
HOMELESSNESS PREVENTION AND RAPID RE-HOUSING PROGRAM (HPRP)
In accordance with the applicable statutes and the regulations governing the consolidated plan
regulations, the state, territory, or local government certifies that:
Affirmatively Further Fair Housing - The state, territory, or local government will
affirmatively further fair housing, which means it will conduct an analysis of impediments to fair
housing choice within the jurisdiction or state, take appropriate actions to overcome the effects of
any impediments identified through that analysis, and maintain records reflecting that analysis and
actions in this regard.
Drug-Free Workplace - It will orwill continue to provide a drug-free workplace by:
1. Publishing a statement notifying employees that the unlawful manufacture, distribution,
dispensing, possession, or use of a controlled substance is prohibited in the grantee's
workplace and specifying the actions that will be taken against employees for violation of
such prohibition;
2. Establishing an ongoing drug-free awareness program to inform employees about:
(a) The dangers of drug abuse in the workplace;
(b) The grantee's policy of maintaining a drug-free workplace;
( c) Any available drug counseling, rehabilitation, and employee assistance programs;
and
(d) The penalties that may be imposed upon employees for drug abuse violations
occurring in the workplace;
3. Making it a requirement that each employee to be engaged in the performance of the grant
be given a copy of the statement required by paragraph 1;
4. Notifying the employee in the statement required by paragraph 1 that, as a condition of
employment under the grant, the employee will -
(a) Abide by the terms ofthe statement; and
(b) Notify the employer in writing of his or her conviction for a violation of a criminal
drug statute occurring in the workplace no later than five calendar days after such
conviction;
5. Notifying the agency in writing, within ten calendar days after receiving notice under
subparagraph 4(b) from an employee or otherwise receiving actual notice of such
conviction. Employers of convicted employees must provide notice, including position
title, to every grant officer or other designee on whose grant activity the convicted
employee was working, unless the Federal agency has designated a central point for the
receipt of such notices. Notice shall include the identification number(s) of each affected
grant;
6. Taking one of the following actions, within 30 calendar days of receiving notice under
subparagraph 4(b), with respect to any employee who is so convicted -
(a) Taking appropriate personnel action against such an employee, up to and including
termination, consistent with the requirements of the Rehabilitation Act of 1973, as
amended; or .

(b) Requiring such employee to participate satisfactorily in a drug abuse assistance or
rehabilitation program approved for such purposes by a Federal, state, or local health,
law enforcement, or other appropriate agency;
7. Making a good faith effort to continue to maintain a drug-free workplace through
implementation of paragraphs 1,2,3,4,5 and 6.
Anti-Lobbying - To the best of the state, territory, or local government's knowledge and belief:
1. No Federal appropriated funds have been paid or will be paid, by or on behalf of it, to any
person for influencing or attempting to influence an officer or employee of any agency, a
Member of Congress, an officer or employee of Congress, or an employee of a Member of
Congress in connection with the awarding of any Federal contract, the making of any
Federal grant, the making of any Federal loan, the entering into of any cooperative
agreement, and the extension, continuation, renewal, amendment, or modification of any
Federal contract, grant, loan, or cooperative agreement;
2. If any funds other than Federal appropriated funds have been paid or will be paid to any
person for influencing or attempting to influence an officer or employee of any agency, a

Authority of Local Government, State, or Territory -- The submission of the consolidated plan
is authorized under state law and local law (as applicable) and the jurisdiction or state possesses
the legal authority to carry out the programs under the consolidated plan for which it is seeking

awards the grant. If it is later determined that the grantee knowingly rendered a false certification, or
otherwise violates the requirements of the Drug-Free Workplace Act, HUD, in addition to any other
remedies available to the Federal Government, may take action authorized under the Drug-Free
Workplace Act.
Workplaces under grants, for grantees other than individuals, need not be identified on the
certification. If known, they may be identified in the grant application. If the grantee does not identify
the workplaces at the time of application, or upon award, if there is no application, the grantee must
keep the identity of the workplace(s) on file in its office and make the information available for
Federal inspection. Failure to identify all known workplaces constitutes a violation of the grantee's
drug-free workplace requirements.
Workplace identifications must include the actual address of buildings (or parts of buildings) or other
sites where work under the grant takes place. Categorical descriptions may be used (e.g., all vehicles
of a mass transit authority orState highway department while in operation, State employees in each

If the workplace identified to the agency changes during the performance of the grant, the grantee
shall inform the agency of the change(s), if it previously identified the workplaces in question (see
paragraph three).

Workplace common rule apply to this certification. Grantees' attention is called, in particular, to the
following definitions from these rules:

"Controlled substance" means a controlled substance in Schedules I
through V of the Controlled Substances Act (21 V.S.C. 812) and as further
defined by regulation (21 CFR i308.11 through 1308.15);
"Conviction" means a finding of guilt (including a plea of nolo
contendere) or imposition of sentence, or both, by any judicial body
charged with the responsibility to determine violations of the Federal or
State criminal drug statutes;
"Criminal drug statute" means a Federal or non-Federal criminal statute
involving the manufacture, distribution, dispensing, use, or possession of
any controlled substance;
"Employee" means the employee of a grantee directly engaged in the
performance of work under a grant, including: (i) All "direct charge"
employees; (ii) all "indirect charge" employees unless their impact or
involvement is insignificant to the performance of the grant; and (iii)
temporary personnel and consultants who are directly engaged in the
performance of work under the grant and who are on the grantee's payroll.
This definition does not include workers not on the payroll of the grantee
(e.g., volunteers, even if used to meet a matching requirement; consultants

Homelessness Prevention and Rapid Re-Housing Program (HPRP)
Certifications
The HPRP Grantee certifies that:
Consolidated Plan - It is following a current HUD-approved Consolidated Plan or
CRAS.
Consistency with Plan - The housing activities to be undertaken with HPRP funds are
consistent with the strategic plan.
Confidentiality - It will develop and implement procedures to ensure:
(1) The confidentiality of records pertaining to any individual provided with assistance;
and
(2) That the address or location of any assisted housing will not be made public, except to
the extent that this prohibition contradicts a preexisting privacy policy of the grantee.
Discharge Policy - A certification that the State or jurisdiction has established a policy
for the discharge of persons from publicly funded institutions or systems of care (such as
health care facilities, foster care or other youth facilities, or correction programs and
. institutions) in order to prevent such discharge from immediately resulting in
.homelessness for such persons.
HMIS - It will comply with HUD's standards for participation in a local Homeless
Management Information System and the collection and reporting of Client-level
information.

WHEREAS, Augusta-Richmond County has availed itself of the provisions of the American
Recovery and Reinvestment Act of 2009; and

WHEREAS, it is necessary to amend the City's 2008 Action Plan as a prerequisite to receiving
Homelessness Prevention and Rapid Re- Housing Program funds after a public hearing has been held
and projects suggested by citizens and organizations have been given careful and serious
consideration; and
I
WHEREAS, in order to expedite the orderly execution of the assurances, certifications and
Grant Agreement, the Mayor is hereby given authority to provide such assurances, certifications and
execute the Grant Agreement as required by the Secretary; and
I
WHEREAS, in order to expedite the federal Homelessness Prevention and Rapid Re- Housing
Program (HPRP), the Housing and Community Development Department is given the authority to
carry out the program as approved by the Augusta-Richmond County Commission; and
I
NOW, THEREFORE, BE IT RESOLVED: That the Mayor and the Housing and Community
Development Department be authorized to amend the 2008 Action Plan, prepare and submit the
HPRP application, thereto as well as provide the required assurances, certifications, and execution of
the Grant Agreement; and

BE IT FURTHER RESOLVED: That the Housing and Community Development Department
be authorized to carry out the orderly execution of the Homelessness Prevention and Rapid Re-
