Attaehed is a detailed chart showing the scope of work for removing several signs ill the
vicinity of the work sites.
THE FOLLOWING RESPONSES ARE PROVIDED TO VENDOR QUESTIONS:
1) Plea.se define exactly what needs to be included in the topa survey? Why is this required
in other words?
Answer: Following demolition, provide topographical survey of site, indicating
locations and sizes of all subsurface, residual foundations, including spread
footings, pile caps and piles, to a depth of at least 42" below finished floor
elevation of the ground floor of the buildings. Provide spot elevations at top of
said foundation structures and statement of elevation datum, using National
Vertical Geodetic Datum (NVGD) 1929. Give location of benchmark used.
2) Is a small business contractor required for this work?
Answer: No
3) Can the fence be transferred from one property to the next, or will the fence have to
remain even after demontian is complete? Is the fence just being used to keep unwanted
individuals out while demolition is in progress?
Answer: The fence is intended to keep unauthorized personnel out of work sites
while demolition and site restoration is in progress.. The contractor may move the
fence from one work site to the next as demolition is completed at each site
4) Where is the Asbestos survey located?
Answer: Copies of appropriate excerpts from the asbestos and lead-based paint
survey report are attached herein.

5) Where can f find a risting of smarr business/minority companies to use as

Answer: It is recommended that you contact the Disadvantage Business
Enterprise Department at (706) 821-2406 or (706) 82.6-1325.
6) Are there any underground tanks on site that require removar?
Answer: No
7) What happens with the large trees on site? Are they required to be removed?
Answer: Trees do not need to be removed unless they interfere with demolition
activities. Any trees that must be removed may be chipped and mulched for use
on site (see response to Question 10 below), disposed of properly at a certified
C&D landfill, or taken to the Augusta Richmond County landfill.
8) Is there any kind of guidelines for recycfing?
Answer: Augusta Richmond County does not provide contractors with guidelines
for recycling.
9) Are basements required to be backfirred?
Answer. Yes - all basements will need to be backfilled to existing grade.

Answer: Each of the demolition sites will need to be stabilized in accordance with
The Manual for Erosion and Sediment Control in Georgia (Green Book).

Answer. Only trees, shrubs, and other foliage that interferes with demolition
activities need to be removed.

1.0 EDCUTIVE S11MMARY
A representative of Alternative Construction" Environmental Soluti~ Inc., peafumaed
an asbestos iDspectioa of 1086 Alexander Drive, Augusta, Georgia 011 April 14,. 2009. The
purpose of the inspectioo was to determine if asbestos-containing materials would be
encountered during demolition of the structure. Random SEJPles of suspect -*m81s
were collected and analyzed for asbescos content
It is ACES's UDderstanding that the structure is sclK:duled for demolition. l1le asbestos
inspedion, therefore, was performed to comply with 29 CFR. 1926.1101.. Duri.ag
demoli~ the identified friable ACM or non-ffiable ACM 1bat might become friable
during demolition must be abated prior to disturbaoce. A State of Georgia licensed
asbestos COJltraetor using a licensed supervisorlforeman and properly trained -.orkers in
aeeordance with a project design 9a~~ by an aceredited projeet designer must pedoun
asbestos abatement. In addition, an air IDODitoring professional should perfotm third-
party air monitoring during. abatement. It is important to note. that Federal and State
replBtions require a lo..day notifica1ion prior to any renovatioDldemolition activities dJat
may impact the condition of ACM in a building that affords public access. or oc:cupanq.
These activities must be performed in aecordance with amad Federal and State of
Georgia guidelines.
1.8 OBJECTIVE
The objective of ACES's inspection was to identify and sample suspect asbestos
c:ontainn.g materials within the structure. This report was prepared for the exdusive use
of the client for specific application. to the subject project. No WBI11IIlties,. either
expressed or implied, are made or intended.. The material CODtained herein shall not, in
whole or part, be disseminated. or conveyed to any other party or be used or relied upm
by any other party, in whole or pert, without ACES's written CODSeI1t.
3.& mE DESCRIPTION
"[his is a brick structure with. a shingled roof The original section of the house is a sin@1e.
story structure with plaster walls. The rear section of the structure is a two story addition.
with a new family room, bath, laundry room, ptage OIl the :first floor and two bedrooms
and a h8throom on the second :floor.
4.8 INSPECTION PROTOCOL
Random samples of suspect 8bestos materials were collected by an U.S. EnvirontQelltal
Protection Agency (USEP A) accredited asbestos inspector. Suspect materials were
grouped into specific homogeneous areas using one of the following classificatious:
surfacing material, thermal system insulation, or misc:el~nr.nus materials. A surfacing
material is sprayed, troweledooOl1, or otherwise applied to surfaces (ie., fireproofin&
ceiliDg texture). Thermal system insulation consists of material applied to pipes, fittTr,p.
boilers,. breaching, tanks, ducts. or other stmetural components to prevent heat loss or

gain. Miscellaneous materials consist of items such as flooring, ceiling tiles. roofing
~ and eement board. In the course of an inspection the inspector mndcmty
collects samples that are assessed for friability, accessibility. condition, da.t:nage potentiaL
and relation to the ventilation System.
The inspector, using a Jmif~ chisel, or coring tool, collected bulk samples of susped.
materials. . Prior to sampling, when required. the suspect ma1erial was sprayed with a
surfactant to reduce fiber release. Samples collected were assigned a unique sample ID
number and placed in a sealed container. Samples were sent to McCall & Spero
EnviromncntaJ, Inc.. a NVLAP accredited laboratory (101895-0) and analyzed for
asbestos con1eDt by Polarized Light Microscopy (PLM) coupled with dispersion dainil1g
tedmiques, in accordance with USEPA Method 40 CFR, Chapter 1, Part 763, Subpart F,
Appendix A. The percentage of asbestos, where applieable. was determined by
microscopic visual e<d:inuttion.
Regulatory agencies (USEPA and OSHA) have defined ACM as a material contaill~
greater than ODe percent asbestos. In order to define a material as non-ACM, a minimum
number of samples must be co11ec:ted and analyzed dependent upou. the type and quantity
of the homogeneous material. Additionally. samples determined to have an asbestos
content of'less than 10 per cent: through PLM visual estimation (iDcluding those rrutWi.
with an asbestos content of less than one peIUIlt) may either be assumed ACM or
verified for asbestos content by point count analysis. A point count analysis. is a
statistical method for quantifying the percentage of asbestos in a material by PLM. Point
counting was not within the scope of work for this project.
Asbestos materials are assessed as fiiable or non-friable. Friable materials. when dry, can
be crumbled. pulverized or reduced to powder by hand pressure and have a higher
pote:ntiaJ. for a fiber release dum non-friable materials. Non-friable materials are
materials that are firmly bound by plamic, cement, etc., and cannot be reduced to powder
by band pressure. The USEPA recommends that floor tile materials with no detectable
asbestos through PLM analysis be verified tbrough Trammi!l,jon Electron Microscopy
(TEM) analysis. Analysis utilizing TEM techniques was not performed at this time..

Thirty samples were collected from the structure. Table lA is a listing of homogeneous
materials for the house and Table lB is a listing of malytical resul1s for asbestos.

6.0 CONCLUSIONS AND RECOMMENDATIONS
Inc:om:lusive results would be results where the Polarized Light Microscopy analysis
yielded a result of less than 1 % ubestos. These tn4ttaials would have to be treated as
asbestos containiqi material or point counted to determine their final status. 'l'hc:re were
no samples ,tbat contained inconclusive results.
In summary, the survey identified fiiable ACM in the structure.' This material includes
(quantities ate approximate):

DmiDg this inspection, materials suspect to contain asbestos may have been identified,
but due to these materials location or current use, were not sampled. These non-sampJed
suspect materials include, but ate not limited to. physically inaccessible materials, and
materials that when sampled would degrade the operation of their parent system or
con1ribute to asbestos comamination. of a building system. Examples of the last category
include gaskets and dampers in ventilation systems, and boiler seals.
It is ACES's understanding the house is scheduled for demolitio.n. Therefore, the
identified asbestos containing materials must be removed prior to any activity tbat would
disturb the material. A licensed asbestos abatement contractor,. usmg a Iiamsed asbesWs
abatement supervisor/foreman and properly trained workers must be employed to
perform the abatement. Additionally, an &f;Credited project d.esipter must design die
abatement project, and third party air monitoring should be performed. during abetemem.
Abatement must be performed in accordance with current Federal USEP~ OSHA, and
State of Georgia guidelines.
This inspection was performed by Mark Hartz, AHERA InspectorIMauagemeDt. Phumer

Specific tasks included are:

2. Prepare a Safety Plan and submit to Owner's Project Manager, within 14 days of executed
Agreement. Plan should include emergency response procedures, discussion of potential
hazards, personaf protective equipment, air monitoring, etc.
3. Attend Pre-Demolition Conference with Owner Representatives..
4. Obtain and pay for all required permits, taking into account all applicable laws and regulations.
5. Prior to demolition work, utilize an approved Pest Control Contractor to search for and
exterminate rodents found on the property.
6. Cooperate and support environmental engineers in their abatement oversight duties, including air
monitoring and abatement compliance inspections.
7. Dispose of aU hazardous waste in accordance with Abatement Plan and Local and State
regulations.
8. Protect aU adjacent properties that may be affected by the demolition work, including historic
homes and operating businesses.
9. Install six foot high chain link fence with two feet of barbed wire on top and two ten foot gates to
secure the site.. Fence shall be placed prior to start of Work and shall remain in place until work
is Substantially Complete, whereupon it shall be removed by the Contractor.
10. Utilize water-based or other approved dust control methods to reduce migration of dust and
debris onto adjoining properties.

1) I'm aware of your statement concerning no questions past June 9, 2009 however,
question (4) in addendum (I) ask, "Where are the asbestos surveys. located?" Answer,
"Copies of appropriate exerts from the asbestos lead-based paint survey report are
attached here-in."
However, our addendum No. (I) contained only part of one re.port. 1086 Alexander Dr.,
doesn't the remaining structures scheduled for demolition require an inspection report for
asbestos and lead paint also? If so, could they be made available.
Answer: The entire asbestos survey report will be provided to the successful
bidder. Each of the ttvee buildings on Alexander Drive contained ACM; neither of
the two on Carriage Creek did.

CONSULTANT SERVICE AGREEMENT
Project: Residential Building Demolition. Five Sites on Alexander Drive and Carriage Creek

This Agreement is made and entered into this day of
between the City of Augusta, hereinafter called the "City" and
to do business in Georgia, hereinafter called the "CONSULTANT."

WHEREAS, the City desires to obtain the services of a qualified and experienced Consultant
to provide certain services as outlined in Attachment "A," of this agreement entitled 'Scope of Work';
said services being related to the Demolition Activities on the above referenced project; and
WHEREAS, the QUALIFIED REPRESENTATIVE OF THE CONSULTANT has
represented to the City that it is experienced and qualified in these activities as identified in said
Attachment "A," and the City has relied upon such representation;
NOW, THEREFORE, in consideration of the mutual promises and covenants herein contained,
it is agreed by and between the City and the CONSULTANT (further reference herein to
CONSULTANT shall also mean to include CONSULTANT; QUALIFIED REPRESENTATIVE of
the CONSULTANT; and, any qualified DESIGNATED REPRESENTATIVE of the
CONSULTANT) that:
1. This Consultant Service Agreement may be entered into with either an individual Consultant,
as a single entity, or with a firm, partnership, corporation or combination thereof.
A. If said agreement is entered into with a firm, partnership, corporation or combination
thereof, the following regulations will apply:
1. The names of the Consultant's designated representatives must be identified on
Page 5 in this agreement and must be approved by the Georgia Department of
Transportation (GDOT) and be fully qualified to perform the required services.
2. The City and the Consultant recognize that continued and uninterrupted
performance of the specified services is essential. Therefore, it is further agreed
between the parties that in the event a named representative of this agreement leaves
the Consultant's employ, the Consultant shall immediately notify the City and
with approval from the City, assign this agreement to an equally qualified
representative.
B. The representatives designated by a firm, partnership, corporation or combination
thereof to perform certain service assignments hereby understand and agree to abide by
all requirements and conditions ofthis agreement as verified by Consultant's signature.

2. Consultant agrees to begin services as outlined in Attachment "A" upon receipt of the Notice
to Proceed issued by the City.
3. It is understood and agreed that the Consultant performing any work under this contract will
not employ, assign, or use any individual in the performance of this contract who is not
pre qualified and not previously approved by the GDOT to perform the services outlined in
Attachment "A."
4. This agreement is not assignable by Consultant, either in whole or in part and no portion of the
work may be sublet or transferred to any other persons without prior written approval of the
City. If it is discovered that such a violation has occurred, the Consultant of record will be
subject to suspension for a period oftime to be specified by the City.
5. It is fully understood and agreed that in the event the services are not completed within the
specified time, a penalty of suspension from all Consultant activity for a period of time to be
specified by the GDOT may be imposed. However, it is understood that an extension of time
may be granted by the City, under extenuating circumstances, if conditions so warrant and the
request for such an extension is submitted in writing.
6. The Consultant further warrants that he has not employed or retained any company or person
other than an individual solely under his employ to solicit or secure this agreement and that he
has not paid or agreed to pay any company or person other than one of his bona fide employees
working solely for him any fee, commission, percentage, brokerage fee, gifts, or any other
consideration, contingent upon or resulting from the award or making of this agreement. For
breach or violation of this warranty, it is understood that the City will have the right to annul
this agreement without liability, and in the event conditions or circumstances so warrant, this
agreement may, by mutual consent, be adjusted as to time or basis or payment.
7. The Consultant will immediately notify the City of any conflicts of interest which exists or
may arise in the Consultant's or its representatives' performance of any activity addressed in
the scope of services of this agreement. Upon said notification, the City will have the option of
either canceling this agreement in its entirety or withdrawing certain service activities as are.
necessary to eliminate the Consultant's conflict of interest.
8. The City shall have the right of cancellation for all or any part of the services under the terms
of this agreement for any reason by giving notice to the Consultant herein, and shall be liable
for services furnished only to the date of receipt of such notice by the Consultant. In the event
of cancellation, Consultant shall be paid a proportionate part of the fee for the particular
services terminated as set forth above in proportion to the work actually completed at the date
of cancellation. It is agreed that if for any reason work under this agreement is terminated; all
work actually done by the Consultant to the date of cancellation shall become the property of
the City.
9. It is agreed that if a dispute concerning a question of fact in connection with the work not
disposed of by this agreement, arises, the decision of the City shall be final.
10. The Consultant agrees to indemnify and save harmless the City, its officers, agents, and
employees from any and all claims and loss occurring or resulting to any and all persons, firms,

or corporations furnishing work, services, materials or supplies in connection with the
performance of this contract, and from any and all claims and losses occurring or resulting to
any person, firm, or corporation who may be injured or damaged by the Consultant in the
performance of the agreement.
11. The parties hereto agree that the Consultant, and any agent of the Consultant, in the
performance of this agreement, shall act in an independent capacity and not as officers,
employees or agents of the City.
12. The Consultant agrees to comply with all Federal, State and local laws and all policies and
procedures of the City applicable to the work.
13. The Consultant also agrees to comply with Federal regulations relative to non-discrimination
in Federally-assisted programs of the U.S. Department of Transportation as herein defined
under Appendix "A" below:
APPENDIX "A"
a. Compliance with Regulations: The contractor shall comply with the Regulations
relative to nondiscrimination in Federally-assisted programs of the Department of
Transportation, Title 49, Code of Federal Regulations, Part 21, as they may be amended
from time to time, (hereinafter referred to as the Regulations), which are herein
incorporated by reference and made a part of this contract.
b. Non discrimination: The contractor, with regard to the work performed by it during the
contract, shall not discriminate on the grounds of race, color or national origin in the
selection and retention of contractors, including procurements of materials and leases of
equipment. The contractor shall not participate either directly or indirectly in the
discrimination prohibited by Section 21.5 of the Regulations, including employment
practices when the contract covers a program set forth in Appendix B of the
Regulations.
c. Solicitations for Subcontractors, Including Procurements of Materials and Equipment:
In all solicitation either by competitive bidding or negotiation made by the contractor
for work to be performed under a subcontract, including procurements of materials or
leases of equipment, such potential subcontractor or supplier shall be notified by the
contractor of the contractor's obligations under this contract and the Regulations
relative to nondiscrimination on the grounds of race, color or national origin.
d. Information and Reports: The Contractor shall provide all information and reports
required by the Regulations, or directives issued pursuant thereto, and shall permit
access to its books, records, accounts, other sources of information, and its facilities as
may be determined by GDOT or the Federal Highway Administration (FHWA) to be
pertinent to ascertain compliance with such Regulations or directives, where any
information required of a contractor is in the exclusive possession of another who fails
or refuses to furnish this information, the contractor shall so certify to the GDOT or the
FHW A, as appropriate, and shall set forth what efforts it has made to obtain the
information.

e. Sanctions for Noncompliance: In the event of the contractor's noncompliance with the
nondiscrimination provisions of the contract, the City shall impose such contract
sanctions as it, GDOT or FHW A may determine to be appropriate, including, but not
limited to:
1) Withholding of payments to the contractor or under the contract until the
contractor complies, and/or
2) Cancellations, termination or suspension of the contract, in whole or in part.
f. Incorporation of Provisions: The contractor shall include the provisions or paragraphs
"a" through "f' in every subcontract, including procurements of materials and leases of
equipment, unless exempt by the Regulations, or directives issued pursuant thereto.
The contractor shall take such action with respect to any subcontract or procurement as
the City, GDOT or FHWA may direct as a means of enforcing such provisions
including sanctions for non-compliance: Provided, however, that in the event a
contractor becomes involved in, or is threatened with, litigation with a subcontractor or
supplier as a result of such direction, the contractor may request the City to enter into
such litigation to protect the interest of the City, and, in addition, the contractor may
request the GDOT to enter into such litigation to protect the interest of the State and
FHW A to enter into such litigation to protect the interest of the United States.
DRUG-FREE WORK PLACE CERTIFICATION
14. The undersigned certifies that the provisions of Code Section 50-24-1 through 50-24-6
of the Official Code of Georgia Annotated, relating to the "Drug- free Workplace Act",
have been complied with in full. The undersigned further certifies that:
(1) A drug-free workplace will be provided for the contractor's employees during the
performance of the contract; and
(2) Each contractor who hires a subcontractor to work in a drug-free workplace shall
secure from that subcontractor the following written certifications: "As part of the
subcontracting agreement with certifies to the contractor that a drug-free
workplace will be provided for the subcontractor's employees during the
performance of this contract pursuant to Paragraph (7) of Subsection (B) of Code
Section 50-24-3."
(3) Also, the undersigned further certifies that he will not engage in the unlawful
manufacture, sale, distribution, dispensation, possession, or use of a controlled
substance or marijuana during the performance of the contract.
15. All information and/or knowledge gained from the performance of this agreement are to
be treated as confidential and privileged communication. The Consultant shall take all
necessary steps to insure that neither he nor any member of his staff or organization
divulges any information gained from the performance of this agreement to anyone
other than the proper officials of GDOT or officials of the FHW A until authorized by

City Officials to do so, or until the Consultant is required to do so by due process of
law or until released from this obligation by having publicly testified as to such
findings.
16. It is mutually understood and agreed that no alteration or variation of the terms of this
agreement shall be valid unless made in writing and signed by the parties hereto, and
that no oral understanding or agreements not incorporated herein, and alterations or
variations of the terms hereof, unless made in writing between the parties hereto, shall
be binding on any of the parties hereto.

SCOPE OF SERVICES
Specific tasks included are:
18. Within 14 calendar days of executed Agreement, submit General Work Plan to Owner's Project
Manager, including scheduled completion of all major tasks.
19. Prepare a Safety Plan and submit to Owner's Project Manager, within 14 days of executed
Agreement. Plan should include emergency response procedures, discussion of potential
hazards, personal protective equipment, air monitoring, etc.
20. Attend Pre-Demolition Conference with Owner Representatives.
21. Obtain and pay for all required permits, taking into account all applicable laws and regulations.
22. Prior to demolition work, utilize an approved Pest Control Contractor to search for and
exterminate rodents found on the property.
23. Cooperate and support environmental engineers in their abatement oversight duties, including
air monitoring and abatement compliance inspections.
24. Dispose of all hazardous waste in accordance with Abatement Plan and Local and State
regulations.
25. Protect all adjacent properties that may be affected by the demolition work, including historic
homes and operating businesses.
26. Install an eight foot high chain link fence with two ten foot gates to secure the site. Fence shall
be placed prior to start of Work and shall remain in place until work is Substantially Complete,
whereupon it shall be removed by the Contractor.
27. Utilize water-based or other approved dust control methods to reduce migration of dust and
debris onto adjoining properties.
28. Completely demolish, remove and properly dispose of all on-site structures, including all
concrete and asphalt parking areas. The removal of this concrete or asphalt should be done in
such a way as to minimize the impact on the surfaces under these materials.
29. Following demolition, provide topographical survey of site, indicating locations and sizes of all
subsurface, residual foundations, including spread footings, pile caps and piles, to a depth of at
least 42" below finished floor elevation of the ground floor of the buildings. Provide spot

elevations at top of said foundation structures and statement of elevation datum, using National
Vertical Geodetic Datum (NVGD) 1929. Give location of benchmark used.
30. Coordinate all utility work with appropriate authorities, including Augusta Utilities, Georgia
Power, BellSouth and the natural gas company (Atlanta Gas Light).
31. Provide traffic control as needed.
32. Contact all necessary utility companies for all appropriate disconnections of service and
arrange for removal of any utility-owned equipment.
33. Remove all rubble and debris from the property, transport and dispose of in accordance with
Local and State regulations.
34. Provide evidence of proper manifests and disposal procedures, including delivery receipts at
permitted disposal facilities. Include type of materials, type/location of facility, date of
delivery and tipping fee paid.
35. Comply with all Federal, State and Local regulations.
All salvageable materials will become the property of the Contractor, unless specifically noted
otherwise.
