CONTRACT BETWEEN AUGUSTA, GEORGIA
AND
SAND mLLS NEIGHBORHOOD ASSOCIATION

2003 HOME INVESTMENT P ARTNERSmpS PROGRAM
THIS AGREEMENT, made and entered into on this~ r ~ay of April 2005, by and
between AUGUSTA, GEORGIA, by and through the Augusta-Richmond County
Commission, as the Implementor of the HOME Investment Partnership Program
(hereinafter referred to as "Grantee"), and SAND mLLS NEIGHBORHOOD
ASSOCIATION (hereinafter referred to as the "Subrecipient") is hereby amended
incorporating the following revisions to said agreement.

APPENDIX A
SCOPE OF SERVICE
Project Description: Sand Hills Neighborhood Association agrees to utilize approved
HOME funds to support continual project related cost associated with the revitalization of
the Sand Hills Community. Under this agreement SHNA will purchase fIve lots to be
utilized for single family homes to eligible families with the remaining funds to be used
as a debt reserve for the 10-unit apartments. The properties are listed as follow:
I) 2420 Mount Auburn Street
3) 0000 Mount Auburn Street
5) 2436 Mount Auburn Street
2) 2422 Mount Auburn Street
4) 2434 Mount Auburn Street
APPENDIX B
Term of Contract
The term of this agreement shall commence on the date when this agreement is executed
by Augusta and the Sand Hills Neighborhood Association and shall end on July 1, 2005.
All required reporting must be submitted within 30 days of the completion of this project.
SHNA must submit in writing if additional time is needed to complete this project.
APPENDIX C
Proiect Proceeds
SHNA shall retain all proceeds. All proceeds must be used for HOME-eligible or other
housing activities to benefit low-income family, as required by CDR 92-300(a)(2). A
separate agreement will be given to SHNA in regard to the use and reporting of all
proceeds. The City will review and/or approve the budget outlined by SHNA for usage
of proceeds.
APPENDIX D
Request for Funds
SHNA may not request disbursement of funds under this agreement until eligible costs
has occurred. SHNA must submit all required documentation along with reimbursement
requests. SHNA must submit a budget for the debt reserve. All reimbursements must
comply with the budget. A HUD-l statement must be submitted along with purchase
agreement for each home.
APPENDIX E
Use of Funds
1. Operating Deficit Reserve
An amount not to exceed $ 36,282.00 shall be expended by SHNA to support an
operating deficit reserve for costs association with the 10-unit Apartment project at 726
Fleming Avenue. The reserve shall be used to meet any shortfall in project income
during the period of project rent-up (not to exceed 18 months) and be utilized for
operating expenses only. SHNA must submit a budget prior to disbursement of any
funds. This budget must be approved by AHED.
2. Acquisition
An amount not to exceed 49,500.00 shall be expended by SHNA to support acquisition of
fIve properties. The acquired sites will be utilized to construct affordable single-family
housing units to be made available for purchase or rent by eligible low and moderate-
income homebuyers. SHNA will acquire the following properties:
1) 2420 Mount Auburn Street
3) 0000 Mount Auburn Street
5) 2436 Mount Auburn Street
2) 2422 Mount Auburn Street
4) 2434 Mount Auburn Street
