2nd ADDENDUM TO THE
FOOD AND SERVICE CONTRACT FOR THE NUTRITION SITE PROGRAM
THROUGH CSRA CENTRAL SAVANNAH RIVER AGENCY
THIS ADDENDUM TO AUGUSTA/RICHMOND COUNTY, RECREATION AND
PARKS ("Addendum"), entered into on or about June 20, 2006 by and between
Augusta/Richmond County Recreation and Parks with offices in Richmond County, GA
("Purchaser") and G.A. FOOD SERVICE, INC., with offices in St. Petersburg, Florida
("Provider") extends Food Service Contract originally entered into on July 1, 2004
("Contract") as follows:
RECITAL
1. On or about July 1, 2004 Purchaser and Provider (collectively referred to as
"the parties") entered a Contract. Said Contract terms commenced July 1,
2004, expired June 30, 2005 ("Service Term"), and provided an option to extend
its term for two subsequent one-year periods.
2. The parties exercised their option to extend Contract, representing the
parties' first renewal extension to Contract beginning July 1, 2005 and expiring
on June 30, 2006 ("First Renewal Service Term Extension").
3. The parties exercised their option to extend Contract, representing the
parties' second renewal extension to Contract beginning July 1, 2006 and
expiring on June 30, 2007 ("2nd Renewal Service Term Extension").
4. Provider desires to extend its Contract with Purchaser and agrees to 2nd
Renewal Service Term Extension as provided forth herein.
5. Pursuant to this Addendum to Contract, the parties agree to the following
pricing:
$4.45 per meal for Hot Meal, Picnic Meal, Shelf Stable and Frozen Meals
beginning July 1,2006 and ending June 30, 2007.
NOW, THEREFORE, in consideration of the mutual promises, covenants, and
provisions contained herein, the parties agree to the following:
1. Subject to funding, the parties will extend their current Contract to provide for a
2nd Renewal Service Term Extension from July 1,2006 through June 30, 2007.
2. The parties agree to the Extension Prices identified in Item 5 above for the 2nd
Renewal Service Term Extension period.
This Addendum shall be effective July 1, 2006.
All provisions to the Contract Addendum and/or any of its attachments shall
conform to the Contract dated July 1, 2004, in all respects.
All provisions in the Contract that are not in conflict with this Addendum are still
in full force and effect and are to be performed at the level specified in the Contract.
By reference herein, this Addendum and all its attachments are hereby made a
part of the Contract.
IN WITNESS WHEREOF, the parties hereto have caused this (two-page)
Addendum to be executed by their undersigned officials, as duly authorized hereunder.

Note that this agreement includes the preliminary planning allocation for meals from the CSRA RDC and that
final allocation numbers are subject to change based on final state allocations. All Site meals are purchased
voluntarily by the local jurisdiction. Paragraph 1 of the attached Cooperative Agreement duly notes the right to
alter final numbers based on funding availability.
*Note: Title III C1 and C2 meals will be reimbursed by the RDC at ninety (90) percent of their value
(Le. $4.45 x 0.9 equals $4.01). Site C1 and Site C2 meals are voluntarily provided by the community
and are not reimbursed by the RDC and therefore not shown in this agreement.
Periodic adjustments may be made during the course of the year to allocate unused meals in the region
to other areas of need or based on reductions in state funds.
