HomeMy WebLinkAboutCOORDINATED HEALTH SEVICES HOMELESSNESS PREVENTION AND RAPID RE HOUSING PROGRAM FIRST AMENDMENTC(OPY FIRST AMENDMENT CONTRACT BETWEEN AUGUSTA, GEORGIA COORDINATED HEALTH SERVICES, INC. HOMELESSNESS PREVENTION AND RAPID RE- HOUSING PROGRAM (HPRP) This First Amendment to the Contract executed by and between the Parties on October 1, 2009, is made and entered into on the 1st day of April 201o, by and between AUGUSTA, GEORGIA, (hereinafter referred to as "GRANTEE "), and COORDINATED HEALTH SERVICES, INC., (hereinafter referred to as "SUBRECIPIENT "). WHEREAS, GRANTEE desires to amend the ORIGINAL CONTRACT to allow a change in beneficiaries from persons /families who are at -risk of being homeless to persons /families who are homeless in accordance with the HPR Program notice published March 19, 2009; WHEREAS, GRANTEE has determined that this amendment is in its best interest and SUBRECIPIENT agrees to said amendment; NOW THEREFORE, it is mutually understood and agreed by and between the undersigned contracting parties to amend that previously executed agreement as follows: (1) Term II. PROJECT, Paragraph 1, is hereby deleted in its entirety and changed to read: "The Grantee agrees to reimburse the Subrecipient in an amount not to exceed Fifty Three Thousand Dollars and no cents ($53,000.00) hereinafter the "Grant" to implement the following project(s): "Good Samaritan — Rapid Re- housing Program ". Subrecipient will operate a Rapid Re- Housing Program designed to provide financial assistance to patients who are homeless. Said project is more fully set forth in Exhibit `A" attached hereto and made a part hereof. (2) Exhibit A, Section I, SCOPE OF SERVICES "Program Delivery ", Paragraph 1, is hereby deleted in its entirety and changed to read: "The programs will operate at 2110 Broad Street, Augusta, Georgia, Monday through Sunday 24 hours a day. Housing related services will be provided annually to 20 (households) eligible program participants with incomes at or below 50% of the area median income." Page I of 3 (3) Exhibit A., Section I. B., SCOPE OF SERVICES "Levels of Accomplishment — Goals and Performance Measures" is hereby deleted in its entirety and changed to read: The Subrecipient agrees to provide the following levels of program services: (4) Exhibit A.
