EXPLANATION OF CHANGE ORDER NO._?
A DRIVEWAY FROM LANEY WALKER TO A PRIVATE PARKING LOT IS TOO STEEP FOR DESIRABLE VEHICULAR
ACCESS TO THE PARKING LOT. THIS CHANGE ORDER WILL PROVIDE FOR THE REMOVAL, REGRADING, REPAVING
AND FINISHING OF ANY DISTURBANCES RELATED TO THIS DRIVEWAY WORK. THE ORIGINAL CONTRACT
DOCUMENTS DID NOT PROVIDE REQUIRED RW/EASEMENT FOR THE DESIRED DRIVEWAY GRADE ALTHOUGH IT IS
NOT AS STEEP NOW AS IT WAS PRIOR TO CONSTRUCTION. THE PROPERTY OWNER HAS DONATED ADDITIONAL
EASEMENT FOR THIS PROPOSED WORK.
ADDITIONAL CONTRACT TIME IN THE AMOUNT OF SEVEN(7) CALENDAR DAYS .IS REQUIRED FOR THIS PROPOSED
WORK. .

RE: LANEY WALKER BOULEVARD
Dear Mr. Thompson
Below is a revised price to reconstruct drive at the office located at 1124 Laney Walker Boulevard.
The drive was constructed according to plans and specifications. Occupant states drive is too steep for
the clientele. Mabus Brothers has made every effort to cover all items that may be encO\D'ltered while
reconstructing "this drive, however, if Mabus Brothers encounters unknown utility conflicts there will be
additional cost. Any conf]jct with utilities will be the ovmers' responsibility.
Mabus Brothers will not be responsible for any structural failures of the brick retaining wall during
construction of the drive. Our price does not include any shoring, bracing or removing and replacing of
the brick retaining wall. Sod has been placed along drive since quote was given. Any sod that is
damaged due to drive removal and has to be replaced will be added to original contact quantity and
invoiced at the bid price. Any additional easement required to reconstruct the drive will be the
responsibility of Public Works. Seven additional contract days will be required for drive
reconstruction.
Price includes removing the drive to obliterate as much of the slope as possible and reconstructing the
drive.
Lump Sum
$9,958.60
Items of construction include the following: bond., traffic controL staking, concrete sawing, removal of
existing drive, regrade existing drive, construct 8" concrete valley gutter and 4" concrete sidewalk.
