The following change is hereby made to the contract for the above project:
Description of Change (for a more detailed description see attached proposal):
Contractor made two additional video inspections from the original plans of the investigation of the existing
well. In the process of the inspections, the well was found to have gravel in the bottom twenty feet of the well
and this material was removed to complete the video investigation of the-entIre_depth. Material was removed
and holes were found in the casing screens in the area of the gravel. The contractor backfilled and capped the
lower segment of the well to eliminate any subsidence into the well casing from thetavel pack region of the
well.
