RE: Water filtration plant improvements - Additional Services (ZEL 9810)
Dear Mr. Hicks:
The construction work at the Water Treatment Plant has proceeded with a vast number of unforeseen
conditions, which have required our involvement for resolution beyond the scope of Basic Services.
These conditions have resulted in numerous change orders to the construction contract. Here is a
listing of the items where we have provided engineering effort to enable the construction work:
1. Change Order No.1, attached, 7 items @ $215,655.30 in construction costs (attached).
2. Change Order No. 2, attached, l5 items @$138,783.51 in construction costs (attached).
3. Change Orders No. 3,4,and 5, attached, 3 items @ $1l,946in con$1;ruction costs (attached).
4. Change Orders No.6, and 7, attached, 6 items @ $140,298 in construction costs (attached).
The total amount of additional construction costs is $506,682.81. The engineering fees for the
original scope of this work computes to 6.7% of the original scope construction cost. We request
compensation at that same rate of for our work in the additional items. The increase requested is
$34,000, which will allow us to complete the project. Your consideration of our request is
appreciated.

Dear Ms. McMillon,
Referring to earlier correspondence (dated May 5, 2000) concerning removal of
lime from Clearwell #3 completed in May 2000. As a matter of record seven (7)
truckloads of waste material were hauled from the site which was documented
with the Resident Project Representative, Mr. Jeff Henderson..
Per our earlier agreement, each truckload was unit priced at $ 49.00 each. The
total sum due for this work is detailed below.

Dear Ms. McMillon,
Per your request we offer a proposal to remove existing lime from Clearwell #3
then load onto AUD trucks to be transported to an off site location (i.e.. Augusta
Sand Plant) as fill material. In order to deposit this material at no charge, it will
have to be mixed with on site material to reduce the existing moisture content.
We offer this proposal for a lump sum price of Seven Thousand, Two Hundred,
Twenty-four Dollars [ $ 7,223.00 1 exactly.
As a part of this proposal it was agreed by AUD (Le., Mr, Bud Hamlin) that any
required mixing material will be supplied by AUD trucks, If AUD so desires, we
can offer hauling of this material at a rate of $49,00 per truckload removed from
the plant.
Please advise the undersigned concerning this matter at the earliest
convenience. Thank you for this opportunity,

measurement the lotallineal feet repaired is 3,216 feet.
Per our proposal dated November 13. 2000 at a unit pricing of $ 4.UO per lineal
foot this yields a total sum of $ 15,436.00. We request this amount be
processed to be available for next month's billing (i,e.. December p~y cycle).

of a 30"pipeline and valve for Aqua South at the Augusta Ga. Potable water facilities
eastern clearwelL The initial plan for the dive was to enler the 30" pipeline to determine
whether or not the valve was operable or if it could be forced open. When we were called
about this job we were infonned that the flow of the pipe would be minimal and that the
flow direction would be against the diver such as a headwind, but this was not the case.
After a quick review of the clearwell and the pipeline underwater I found that the Dow of
the line was stronger than previously believed and that the flow was entering the pipeline
instead of exiting. In an attempt to penetrate the pipeline I learned that the inside of the
pipe was covered with a very slick algae like substance which would make it virtually
impossible (or me to push myself backward out of the pipe ifa problem should occur. I
came to the conclusion that the dive was unsafe under these conditions was consequently
not feasible.
After the dive was aborted, the workers on site were directed to open and close the
valve repeatedly to try and break it free so that it could close completely. The valve was
opened and closed several times until we felt that it bad broken free, and was closable,
The valve was put into the closed position, and another dive was done, to determine if the
flow had stopped. There was no perceivable flow going or coming from the pipeline. We
were then asked ifwe could go into the pipeline and check the valve, to make sure it had
closed completely. This was considered not feasible for diver safety reasons. There was
no way to determine if the valve in question was under differential pressures((this is one of
the leading causes of fatal accidents in the commercial dive industry) see ADC video
included in the correspondence]. Everyone on site agreed that it was likely that there was
a 90 degree turn in the pipe between the entry point and the valve in question, and this
posed another potential diver hazard.

Per your request we offer to remove the brick veneer on Clearwell #5 to allow for
your inspection of the wall along Iris Drive for the lump sum price of Nine
Hundred Fifty-three Dollars [ $ 953.00 ] exactly. This price allows for the
removal of a 6-8 foot diameter hole in the veneer allowing for a toothed edge to
enable subsequent repair,

equivalent brick (Le" Waters Edge by 80ral) used on the repair at Clearwell #2
for the lump sum of One thousand, One Hundred Forty-four Dollars

Per your request we quote the additional repair of the vertical expansion joints in
Clearvvell #5 for the lump sum of One Thousand, Eight Hundred Eighty-one
Dollars [ $ 1,881.00 J exactly, This repair involved the repair of 28 lineal feet of
expansion joint and approximately 4 feet of horizontal crack. All work was
completed using the materials and procedure (Le" GACO-FLEX) previously
approved for the base contract.
