The following change is hereby made to the contract for the above project:
Description of Change (for a more detailed description see attachment): CSRA Testing and Engineering was
awarded a contract to provide soils testing and materials testing services for 2010 under RFQ 09 -177 Contract
for $34,788. Due to unanticipated expenditures for soils and materials testing requirements for projects
including the Goodrich Street Pumping Station upgrades, Messerly Wastewater Treatment Plant upgrades,
Highland Avenue Water Treatment Plant upgrades, and materials testing for the new Construction and
Maintenance Facility on Wylds Road it is necessary to request additional funds for the CSRA Testing and
Engineering Contract RFQ 09 -177. The funding amount requested to meet payment obligations anticipated for
the remainder of 2010 contract is determined to be $18,740.00. Your approval signature indicates approval of
this change to the RFQ 09 -177 contract in the amount of $18,740.00.
