The following change order is hereby made to the proposal for the above project: a) Install additional grilles and diffusers in accordance with the Mechanical Punch List $ 945.00 b) Install sheet metal cover over exposed piping: in the elevator machine room install an acoustical ceiling including lowering of the smoke detector in accordance with the requirements of the Elevator Inspector $ 770.00 c) Install signage per the schedule prepared bv David O'Neal ofHBOL $1,597.00 d) Install four (4) 4" square pipe bollards with base plates and caps around the HV AC compressor units $ 935.00 TOTAL AMOUNT OF THIS ORDER: $4,247.00 The contract time will remain the same since no additional time is needed for this work. The completion date remains 9-03-04. EXPLANATION: Item A: This item includes installation of grilles and diffusers in the restroom doors in the second floor offices. This is necessary to relieve excessive resistance to the ventilation fan motors. Failure to act would result in significant maintenance cost over the years due to premature failure of the fan motors. Item B: Installation of the metal covers over exposed pipes and an acoustical ceiling in the elevator equipment room are items required by the State Elevator Inspector in order to meet requirement to obtain a valid Certificate of Occupancy. Item C: Signage identifying the rooms in the building was not included in the bid. This adds necessary identifying signage to the rooms in the building including signs to identify the restrooms in accordance with the schedule prepared by David O'Neal ofHBOL. Item D: This item is to install 4" square tube bollard to protect the HV AC units at the south end of the building. Presently they are exposed and vulnerable to damage by vehicular traffic.
