The following modification(s) to the Contract are hereby ordered (use additional pages if required):
1. Patch the defective area on Papaya Street by removing the existing asphalt pavement and replacing it with 4" of
asphalt binder and 2" of asphalt topping mix.
2, Patch the defective area on Papaya Street north of the storm drain near the intersection of of Castlewood and
Papaya Street by removing the existing asphalt pavement and replacing it with 4" of asphalt binder and 2" of asphalt
topping mix.
3. Remove and replace approximately 54 linear feet of 24" concrete curb and gutter.

1, The sub-base on Papaya Street is inadequate and has failed due to the normal traffic loading, The areas of repair are
outside the areas influenced by the construction of the 60" Raw Water Line,
2. The asphalt pavement failed as a result of the storm drain overflowing during a rare heavy storm event. The storm
drainage system was unable to handle the storm runoff flow, damaging the pavement The storm drain was unable to
handle the flow in part due to the fact that sand and silt blocked it. This sand and silt was in part deposited in the storm
drain as a result of a water line break south of the storm drain. The water line break was determined to be partially the
responsibility of the contractor and partially a random break.
3. The concrete curb and gutter along Papaya Street was in poor condition prior to the beginning of construction. Some of
the curb has been replaced as a result of construction activities, while other portions have not been replaced. This
section of curb and gutter is particularly bad and is in need of replacement.
