SUPPLEMENTAL AGREEMENT NO. 4

WHEREAS, We, W. R. Toole Engineers, Inc., Consultant, entered into a contract with
Augusta-Richmond County on October 5, 1999, for engineering design services
associated with the Bungalow Road Improvements Project, Project No. 323-04-
299823595, File Reference No. 03-014 (A), and
WHEREAS, certain revisions to the design requested by Augusta-Richmond County are
not covered by the scope of the original contract, we desire to submit the following
Supplemental Agreement to-wit:
Additional Engineering services required to incorporate plans with approved
revisions made and to provide compiled easement map incorporating new easement as
per W. R. Toole letter dated July 8, 2003.
It is agreed that as a result of the above modification the contract amount is
increased by the amount of $ 1",700.00 from $99,780.00 to $101,480.00.
Any modifications to submittal dates shall be as identified in the attached proposal. This
agreement in no way modifies or changes the original contract of which it becomes a
part, except as specifically stated herein.
NOW, THEREFORE, We, W. R. Toole Engineers, Inc., Consultant, hereby agree to said
Supplemental Agreement consisting of the above mentioned items and prices, and agree
that this Supplemental Agreement is hereby made a part of the original contract to be
performed under the specifications thereof, and that the original contract is in full force
and effect, except insofar as it might be modified by this Supplemental Agreement.

Dear Mr. Edwards:
W.R. Toole Engineers, Inc. (WRTE) appreciates this opportunity to provide you with a
cost proposal for additional professional services for the Bungalow Road Improvements
Proj ect.
As discussed in the meeting on June 12, 2003, and subsequently outlined in the meeting
minutes, WRTE will provide the following services for the Bungalow Road
Improvements Project:
. Remove all revisions from Sheet #2 that occurred prior to date of acceptance by
Richmond County and provide one (1) revised Sheet #2 to be inserted into
original set of construction plans.
. Revise Sheets #21, #24, #50, and #53 to reflect the proposed striping on Peach
Orchard Road as shown on the plans prepared by Day & Wilburn Associates for
the Georgia Department of Transportation. Sheets to be revised to also show
proposed traffic signal loops and strain pole located in new easement area.
WRTE will provide one (1) copy of each revised sheet to be inserted into the
original set of construction plans.
. Provide compiled easement map depicting new easement located on Parcel 107,
Tax Map #98-2.
. Augusta Richmond County Traffic Engineering Department to provide revised
Sheets #54 through #59 reflecting the proposed traffic signal loop and strain pole.

WRTE will provide the above servIces and deliverables for the lump sum fee of
$1,700.00.

associated with the Windsor Spring Road Improvements Project, Project No. 323-04-
296823516, File Reference No. 03-014 (A), and
WHEREAS, certain revisions to the design requested by Augusta-Richmond County are
not covered by the scope of the original contract, we desire to submit the following
Supplemental Agreement to-wit:
Additional Engineering services required to revise plans and incorporate striping
changes and other approved changes per W. R. Toole letter July 8, 2003.
It is agreed that as a result of the above modification the contract amount is
increased by the amount of$I,300.00 from $114,800.00 to $116,100.00.
Any modifications to submittal dates shall be as identified in the attached proposal. This
agreement in no way modifies or changes the original contract of wl)ich it becomes a
part, except as specifically stated herein.

inserted into original set of construction plans.
· Provide Augusta Richmond County Public Works with one (1) revised sheet #2
outlining revisions/dates to be inserted into the original set of construction plans.
WRTE will provide the above services and deliverables for the lump sum fee of$I,300.00.
