WHEREAS, We, Lowe & Associates, Inc., Consultant, entered into a contract with Augusta-Richmond
County on November 6, 2002 for engineering design services associated with the Washington Road
Intersection Improvements Project, Project No. 324-04-201824371, File Reference No. 03-014 (A), and
WHEREAS, certain revisions to the design requested by Augusta-Richmond County are not covered by
the scope of the original contract, we desire to submit the following Supplemental Agreement to-wit:
Additional Engineering services required to conduct a Phase II Subsurface Investigation to
assess an environmental condition that would potentially present a threat to the project site and provide
an Environmental Assessment Report. See attached proposal dated July 15, 2003.
It is agreed that as a result of the above modification the contract amount is increased by the
amount of$4,800.00 from $53,548.81 to $58,348.81.
Any modifications to submittal dates shall be as identified in the attached proposal. This agreement in
no way modifies or changes the original contract of which it becomes a part, except as specifically
stated herein.
NOW, THEREFORE, We, W. R. Toole Engineers, Inc., Consultant, hereby agree to said Supplemental
Agreement consisting of the above mentioned items and prices, and agree that this Supplemental
Agreement is hereby made a part of the original contract to be performed under the specifications
thereof, and that the original contract is in full force and effect, except insofar as it might be modified
by this Supplemental Agreement.

Dear Mr. Chalmers:
Gallet & Associates Inc. (Gallet) appreciates the opportunity to submit this proposal and cost estimate for
your review. This proposal has been prepared to provide a Limited Phase II Investigation on the property
referenced above in response to your request-for-proposal.
Gallet performed a Phase I Environmental Site Assessment (ESA) for the proposed road improvements
(road widenings) along Washington Road, Boy Scout Road, and Center West Parkway, near the intersection
of Washington Road and Boy Scout Road, in Augusta, Richmond County, Georgia (report dated December
18, 2002). According to this Phase I ESA, a review of specific environmental regulatory listings indicated a
facility that would potentially present an environmental threat to the subject site. The facility is located east-
northeast of the subject site along Washington Road, and is listed in the Georgia Environmental Protection
Division (GA EPD) Leaking Underground Storage Tank (LUST) database as follows:

According to the Environmental Data Resources, Inc. (EDR) records review and the GA EPD, a confirmed
release of a petrolewn product was reported on February 24, 1994 at the GA-069 (Crown Central
Petrolewn) facility. The facility is currently under a corrective action plan (CAP-B), and a progress report
was expected at the GA EPD at the end of December 2002. However, no further information was available
at the time the Phase I ESA was completed. In addition, due to the topography at the facility, the
groundwater is expected to flow in a westerly and southwesterly direction (towards the subject site). Based
on the lack of information available on the regulatory status of the GA-069 facility, the prevailing
groundwater flow, and the close proximity of the facility to the subject site, the GA-069 (Crown Central
Petroleum) facility is considered a recognized environmental condition in connection with the subject site.
The purpose of this Limited Phase II Investigation will be to gather information regarding the possible
presence of petrolewn constituents in the groundwater near the subject property.

LIMITED PHASE II SUBSURFACE INVESTIGATION
Field activities conducted will include the following:
1. Gallet will conduct a subsurface investigation to investigate on-site soil and groundwater quality. Four (4)
borings will be advanced (near the eastern edge of the subject site) using Geoprobeā¢ technology, direct-
push sampling methodology. The location of the borings will be determined in the field by the environmental
professional. Groundwater sampling will be performed at each of the boring locations, to a maximum of 30
feet or until groundwater is encountered.
2. As a field indicator, soils will be screened using on-site headspace analysis of organic vapors by method
of a photoionization detector (Pill). The screening process provides field data to help determine if soil has
been impacted by petroleum from past site usage.
3. Groundwater samples will be collected and submitted to an accredited independent laboratory, for
analysis of constituents related to on-site and off-site activities in accordance with Georgia EPD
requirements. Parameters include: Volatiles including Benzene, Toluene, Ethylbenzene and Xylene (BTEX)
using EPA Method 8260, and Semi-Volatiles including Polyaromatic Hydrocarbons (PARs) using EPA
Method 8270.
Upon completion of the fieldwork and laboratory analysis, an environmental assessment document will be
prepared and will include fmdings, conclusions, and recommendations.

We are prepared to initiate the project immediately upon your written authorization to proceed. We
anticipate the field program to commence within I week of written authorization. A verbal report of the
analytical laboratory results will be provided within 1 week of field activities. Three copies of the final report
will be submitted following our receipt of laboratory data. If another schedule is required, we are flexible to
your specific needs.

Underground utility location will require a 72-hour notice prior to the subsurface investigation. We ask
that you obtain permission from the owner for access and subsurface investigation. Also, because this
investigation will entail advancing borings on the roadways or the roadway's righ-of-ways, we ask that
you obtain permission from the appropriate government agency (i.e. Georgia Department of
Transportation or the City of Augusta).
