The City's 2001 program year began on January 1, 2001, Enclosed is the Annual Action Plan Grant Agreement which incorporates the CDBG Entitlement Agreement (HUD 7082), HOME Investment Partnership Agreement (HUD 40093), and Funding Approval for the Emergency Shelter Grants (ESG) Program (three copies each) for your signature, This constitutes the contract between the Department of Housing and Urban Development (HUD) and the City of Augusta, Georgia, You should note any special conditions included in the Funding Approvals (Item 8 for CDBG and ESG Programs and Item 11 for HOME Program). Please retain one copy for your records and return the other two copies to this office. Failure to execute and return the grant agreement within 60 days of the date of this letter may be deemed to constitute rejection of the grant and cause for HUD to determine that the funds are available for reallocation to other grantees. In addition to the Grant Agreement we have also enclosed a copy of "Annual Action Plan Advice and Guidance" applicable to the various Programs. The Office of Fair Housing and Economic Opportunity (FHEO) has stated a concern that the City has not fulfilled its commitment to affirmatively further fair housing as it relates to the disparities identified in the Analysis of Impediments to Fair Housing that was completed in 1995, We encourage the City to seriously consider developing and implementing substantive measures to combat fair housing impediments, Your staff may also contact the Office of FHEO to request assistance in this area, i ANNUAL ACTION PLAN GRANT AGREEMENT CITY OF AUGUSTA, GEORGIA This Grant Agreement is made by and between the United States Department of Housing and Urban Development (HUD) and the City of Augusta. Georgia, Grantee. Emergency Shelter Grants (ESG) Program Grant Number: S-01-MC-13-0004 TIN: 58-6000513 The ESG award which is the subject of this part of this Agreement is authorized by Subtitle B of Title IV of the Stewart B. McKinney Homeless Assistance Act, 42 V.S.C. 11301 (1988), as amended (the "Act"). In addition, the grant operates through HUD's regulations at 24 CFR Part 576, as now in effect and as may be amended from time to time, which are incorporated as part of this Agreement. In reliance upon the Consolidated Plan and certifications, the Secretary agrees, upon execution of the Grant Agreement, to provide the Grantee ESG funds in the amount of $99.000 as provided in the attached HUD Funding Approval form, which constitutes a part of this agreement. The grantee must comply with requirements for recordkeeping and annual performance reporting to HUD within 90 days after the close of its consolidated program year, as required by 24 CFR 91.520. This includes the periodic information collected through HUD' s Integrated Disbursements and Information System (IDIS). The grantee's IDIS reporting must include information on grant activities, project sponsors, project sites, and beneficiaries (including racial and ethnic data on participants). This information will be used for program monitoring and evaluation purposes. The Grantee agrees to comply with all applicable laws and regulations in distributing funds provided under this Grant Agreement and to accept responsibility for ensuring compliance by subrecipient entities which may receive funding assistance. The Grantee agrees to comply with the provisions of the environmental requirements of 24 CFR Part 58 as applicable under 24 CFR 576.57(e) with respect to funds provided under this Grant Agreement. The HOME statute imposes a significant number of data collection and reporting requirements. This includes information on assisted properties, on the owners or tenants of the properties, and on other programmatic areas.
