Standard Form of Agreement Between Owner and Architect
with Standard Form of Architect's Services

ADDENDUM DATE: October 20,2000
A. This Addendum shall form a part of the contract document equally as if bound into the contract.
B. The Owner's representative and Architect shall acknowledge receipt and acceptance of this
Addendum in the space provided below.
C. The contract previously issued and dated March 6, 2000 shall be modified and/or supplemented as
hereinafter set forth.
ITEM 1
Page 1, Paragraph under "For the following Project"; Delete the last sentence and insert
the following: "The budget for the facility is One Million Five Hundred Seventy Five
Thousand ($1,575,000.00) dollars."
ITEM 2
Page 2, Article 1.1, Paragraph 1.1.2.5(.1) and (.2); Change the amounts listed at the end
of each sentence respectively to read: "$1,575,000.00 and $1,453,000.00
ITEM 3
Page 3, Article 1.1, Paragraph 1.1.2.6; Change second line to read "Contract Documents
review date - "March 30, 2001.
ITEM 4
Page 9, Article 1.5, Paragraph] .5.], Change the Stipulated Sum amount to read: One
Hundred Twenty Two Thousand and 00/1 00 dollars.
