Mary Ann Gibbs Ms. Donna Williams Mr. Gerard Boyle FIRST AMENDMENT TO HEALTH SERVICES AGREEMENT This Amendment to the Health Services Agreement by and between Prison Health Services, Inc. (hereinafter referred to as "PHS") and Augusta, Georgia (hereinafter referred to as "Augusta, Georgia"), is dated this 2nd day of July , 2002. Augusta, Georgia and PHS shall hereinafter collectively be referred to as the Parties. WHEREAS, the Parties executed a Health Services Agreement dated April 10, 2001 (the "Agreement"), by which PHS assumed the responsibilities for provision of reasonably necessary medical care to be delivered to the inmates of the Augusta- Richmond County Detention Facility, Augusta-Richmond County Law Enforcement Center and Augusta-Richmond County Correctional Institution (hereinafter called "Facilities"); and WHEREAS, the Parties desire to amend said Agreement; and NOW, THEREFORE, in consideration of the mutual covenants herein contained, the receipt and sufficiency of which are hereby acknowledged; the Parties hereby agree to the following: 1. Effective July 1, 2002, PHS will expand the mental health services it provides to the facilities by adding a fulltime (40 hours per week) social worker and eight hours per week of psychiatric physician services; and 2. Augusta, Georgia agrees to pay PHS an additional $61,700.00 for the expanded mental health services for the period July 1, 2002 through December 31, 2002; and . . 3. In all other respects, the terms and conditions of the Agreement shall continue unchanged and in full force and effect. IN WITNESS WHEROF, the Parties hereto have executed this First Amendment to the Agreement as of this 2nd day of July ,2002.
