WHEREAS, We, Heath & Lineback Engineers, Inc., Consultant, entered into a contract with Augusta-
Richmond County on February 7,2000, for engineering design services associated with the St. Sebastian
Way/Greene Street/15th Street, Project 324-04-202824003 - DE-OOMS (389), File Reference No. 08-
014 (A), and
WHEREAS, certain revisions to the design are not covered by the scope of the original contract, we
desire to submit the following Supplemental Agreement to-wit:
Revisions extending the work on 15th Street from Broad Street to Reynolds Street;
design of 14 additional retaining walls to minimize impacts to properties; GDOT
mandate to use a preapproved street lighting consultant for final street lighting
plans; design of traffic signals at 3 additional intersections; and the extra work
referenced above has created additional expenses for final shop drawings.
It is agreed that as a result of the above described modification the contract amount is increased by
$64,580 from $1,408,931.24 to a new total 1,473,511.24.
Any modifications to submittal dates shall be as identified in the attached proposal. This agreement in no
way modifies or changes the original contract of which it becomes a part, except as specifically stated
herein.
NOW, THEREFORE, We, Heath & Lineback Engineers, Inc., Consultant, hereby agree to said
Supplemental Agreement consisting of the above mentioned items and prices, and agree that this
Supplemental Agreement is hereby made a part of the original contract to be performed under the
specifications thereof, and that the original contract is in full force and effect, except insofar as it might be
modified by this Supplemental Agreement.

BE IT ORDAINED by the Commission-Council of Augusta-Richmond County,
Georgia that the following Capital Project Budget is hereby adopted:
Section 1: The project change is authorized to CPB#324-041110-202824003. The funding for this
project is for revisions extending the work on 15th Street from Broad Street to
Reynolds Street; design of 14 additional retaining walls to minimize impacts to
properties and 2 additional bridges. Funding for this project is available in SPLOST IV.
Section 2: The following revenues are anticipated to be available to the Consolidated
Government to complete the project.
