TillS AGREEMENT made the 1st day of May, 2003 between CONCERN: EMPLOYEE
ASSISTANCE PROGRAM, 961 Broad Street, Augusta, Georgia 30901, hereinafter designated
"CONCERN", and THE AUGUSTA-RICHMOND COUNTY COMMISSION hereinafter designated
ARC.
Whereas, CONCERN provides assistance to business and governmental organizations located in its
geographic service area in connection with the design, implementation, and maintenance of Employee
Assistance Programs for employees of such businesses and organizations.
Witnesses that for and in consideration of mutua] promises herein attached; it is understood and agreed
as follows:
1. The CONCERN: EAP program will consist of:
· Counseling services for employees and their immediate family members provided at
EAP offices, 961 Broad Street, Augusta, GA, Monday - Friday, 8:30 a.m. - 5:00 p.m.,
or at other satellite locations, by appointment.
· Included in this mode] are a maximum of five (5) visits per employee, or eligible
family member, per year for the purpose of assessment, referral, crisis intervention or
short term problem focused counseling.
· Supervisory training will be provided at mutually agreeable times and locations.
· Te]ephone and/or on-site consultations with management and supervisors are
included, also mental/nervous and alcohol and drug assessment and referral to
appropriate network providers or conununity resources.
· Assistance with .promotion of the program through an internal promotional campaign
consisting of posters, flyers, take-home literature and articles for company newsletter,
quarterly general employee newsletter, and monthly supervisory newsletter.
· Quarterly demographic and utilization reports are included.
. Quarterly on-site liaison meetings are provided.
· Additional services are available at varying rates.
2. CONCERN agrees to maintain malpractice liability insurance ($],000,000).
3. The rate will be $1.25 per employee per month, based on the number of employees as of
May 1, 2003 to be paid in monthly installments. The cost in subsequent years will be
based on the total number of employees employed as of May 1st as verified with the
Human Resources Department.
4. The term of this Agreement shall conunence on the Effective Date first stated above and
shall continue for a period of two (2) years, unless otherwise terminated as set forth
below. Unless terminated in accordance with this Section, this Agreement shall renew
automatically for successive annual terms.
5. This agreement may be terminated, or adjustments made at any time, by either party,
upon thirty (30) days written notice to the other party.

Employee Assistance Program
EAP Consultants, Inc. proposes to provide Augusta-Richmond County with a comprehensive
employee assistance program that covers both its 2,600 employees and their dependents. Below is
the cost per employee per month:
· $1.30 for a 3 session per problem EAP*
· $1.95 for a 6 session per problem EAP*
· $2.55 for a 8 session per problem EAP*
Employees and each of their dependents are entitled to the contracted number of sessions each
year. Services included in this cost are the following:

· 24 hour per day, 7 day per week, toll-free EAP access to mental health professionals
· Multilingual interpreter services available in 140 languages with AT&T Language Line
· Counseling offices located conveniently to your employees
· Face-to-face assessment, short term counseling, referral and follow-up for alcohol and
drug problems, work related difficulties, marriage and family issues, emotional and
psychological problems, and other personal concerns that may affect employee well-
being and job performance
· Case management that includes coordinating referrals to resources beyond the EAP,
when needed
· Crisis management to assist with emergencies in the workplace
· Unlimited critical incident stress debriefings conducted on-site for traumatic incidents

· Child care information and referrals for all types of child care, camps and schools.
· Eldercare information and referrals
· Adoption resources
· Legal consultation provided by attorneys. Simple wills prepared at no cost. Twenty-five
percent discount off attorney's hourly rate for legal services rendered beyond the EAP.
· Financial consultation by experts regarding debt matters, investment options, money
management, and retirement planning.
· Academic resources, including customized profiles for military schools, boarding schools
and colleges. Our college planning guides include sources of financial assistance,
educational consultants, SAT information and relevant web sites.
· Tutoring referrals for children needing academic assistance. Clients can receive
information on programs that provide tutoring at a designated site or in the home.
· Pet care services that offer a customized listing of breeders and groomers, walkers and
sitters, kennels, veterinarians, and pet publications. Our Pet Service Guide includes
comprehensive information on pet care.

· Legal/Financial online resource library. This service makes available numerous articles
providing information on important legal and financial matters. Also included is a Legal
Document Library that provides examples of about 50 different legal documents.
· HELPNET is our online EAP and Work/Life program. It offers information, assistance
and resources for fanilly & caregiving, emotional well-being, health & wellness and daily
living. The cost of HELPNET is an additional 25 cents per employee per month.

· Unlimited consultation to supervisors and managers regarding employees with job
performance, attendance and conduct problems
· Monitoring of progress in EAP and treatment for up to two years, when an employee is
referred by management for job/substance abuse problems, by maintaining contact with
the employee, treatment providers, and the employer, if appropriate
· Unlimited consultation regarding organizational issues, including sexual harassment,
drug-free workplace, downsizing, conflict resolution, and workplace violence

· Promotional materials including EAP brochures, customized with your logo and wallet
cards for all employees, quarterly newsletters for all employees, quarterly posters, e-mail
announcements, Frontline Supervisor, a monthly newsletter for supervisors (l copy or
emailed), and Supervisory Guide Books

· Employee orientations, supervisory training, and workllife seminars are $100.00 per
hour, including travel time. Local trainers are used. Employee orientation and
supervisory training videotapes are $20.00 each.
· An Internet-based version of our employee orientation is available at our website at no
cost.
· Financial seminars are available, at no cost in urban areas on such topics as money
management, annual tax changes, retirement planning and saving for college.
