Enclosed is the language for the Letter of Agreement between your government and the Carl
Vinson Institute of Government for the Mystery Shopper Program. Please re-type this language

on your county/city letterhead and secure the signature of your City Mayor or your County
Commission Chair. Please then return the original copy of the LOA to the Vinson Institute in the
enclosed envelope. It is important that the Institute receive an original copy, as University of'
Georgia regulations require an original signature in order to process the LOA.

This letter is to request that the University of Georgia, through the Vinson Institute of
Government, provide "mystery shopping services" via Shop'n Chek, Inc., for the purpose of measuring
customer service as conducted by the Government staff within the various selected offices/departments
being shopped. We would like for the Vinson Institute to: (l) present specific instructions to Shop'n
- Chek regarding which selected offices/departments to shop and/or call within the Government; (2) mail
to the Government contact the Walk-In and Telephone Evaluation Forms to be used by Shop'n Chek
during the respective shops; (3) coordinate with Shop'n Chek to conduct ten shops and/or telephone
calls per each of the Government's selected offices/departments during the six-month shopping period to
take place during July through December, 2000; and (4) forward, from Shop'n Chek to the Government,
the evaluation reports generated after each of two three-month shopping intervals analyzing the results of
the shoppers' visits/telephone calls, plus' the evaluation,questionnaires completed by the'shoppers.
This work will begin on July I and be. completed by December 31. The City of Augusta-
Richmond County will pay the University of Georgia the amount of $6,040, which covers 120 site shops,
plus $100 one-time set-up fee for those Governments signing up for the first time. This amount will be
due on December 31. The invoice will be sent to:
