As provided for in Section 5, Modification, of the Administrative Services Agreem` ”
between Georgia Administrative Services, Inc. (GAS) and City of Augusta/Richmond
County (the Client) dated August 1, 2004, and revised on August 1, 2007, and revised on
August 1, 2010 said agreement to read as follows:
Paragraph 5, Compensation is herein amended to read as follows: GAS will administer
Client's workers' compensation claims program for the fee of $ 232,833 for a three year
contract period beginning August 1, 2010. This fee will be divided in thirty -six
installments and billed monthly. Payment is expected within 15 days of invoice. Further
changes in the fee schedule will be handled by addendum as provided in Section V.
IN WITNESS WHEREOF, the parties have set their hands and affixed their seals for the
