In doing this they also identified each cell of the reservoir as a separately numbered dam. These are now identified as Highland Avenue Water Treatment Plant Pond Dam #1 East and Highland Avenue Water Treatment Plant Pond Dam#2 West. Consequently,we will separately inspect and issue a report on each. However, subsequent discussions with Safe Dams personnel have revealed that a single EAP and a single O&M Manual can be submitted covering both dams. A. Preliminary In May of 2018 we visited the reservoir to visually inspect a small sinkhole that had formed near the granite perimeter edging of the impoundment. This was thought to be a localized loss of material during rainfall events with the material being carried into the reservoir water. It was decided that this would be monitored and then repaired during a drier period of the year. This issue will be included in the inspection and repair plans described below. There has also been discussion that the determination of the height and storage capacity of the two reservoir cells by Georgia Safe Dams personnel may not take into account that the reservoir was partially excavated below existing grade. Consequently, we will initially recalculate the storage in the two cells using available old plans and existing mapping. We will similarly use LiDAR topographic information to re-figure the height of the embankment around the perimeter of the reservoir. Should we find that the height and storage capacity of the reservoir are both under Georgia Safe Dams threshold values,we will make a request on your behalf that the facilities be reclassified. B. Bi-Annual Inspection Based upon the size and/or impoundment capacity of the dams at your reservoir for the Highland Avenue Water Treatment Plant, the dams are deemed a Class I structure and fall under the regulations of the Safe Dams Program of the Georgia Department of Natural Resources, Environmental Protection Division(EPD). In the past EPD would periodically inspect the dams and issue a subsequent letter to you highlighting any deficiencies which need to be addressed. Now the Safe Dams Program is turning some periodic inspection over to dam owners with the inspection results being sent to the Safe Dams Office. About seventeen years ago the Safe Dam Program implemented a requirement that any engineer who wished to work on a Class I dam in Georgia submit detailed records showing experience in dam inspection, design, and construction so that the Program could give qualified individuals the designation of"Engineer of Record." Please note that our firm has two Engineers of Records on our staff. In addition,we have five other professional engineers with dam design experience. We propose to make a visual inspection of the two reservoir dams with one of our Engineers of Record to assess the condition of the dam,to take photographs of pertinent features or conditions, and to identify any deficiencies that might be apparent. This inspection would be based in general upon the Safe Dams Rules and specifically on the Embankment(Earth)Dam Inspection Form issued by the Safe Dams Program. We would then issue to you a report on each dam summarizing our findings. In preparation for making the field inspection we would expect to be provided with any correspondence with the Safe Dams Program regarding previous inspection by their personnel. By reviewing the previous findings, we can be more certain of monitoring any issues already identified. C. Design of Improvements We believe that at a minimum, repairs to the embankments that will be needed include repair of the sinkhole mentioned above, the removal of woody vegetation from the slopes, the repair of miscellaneous 19 OF 29 REVISION DATE: June 2011 0Lsrq r'= u¢: CQOR01P erosion gullies, and the reworking of piping which handles surface drainage. Others may be identified in the condition inspection. The Safe Dams Program publishes Engineering Guidelines which sets forth requirements for the use of design engineers and owners in making submittal for permit approval to build,or make improvements to, a dam. The Guidelines set the minimum standards for the information to be shown on plans and in specifications. They also require that the plans and specifications be accompanied by a Design Report which includes all calculations and design decisions associated with the proposed work. We propose to use the two-foot contour map available on the Augusta GIS website as the base map for the design work supplemented by field surveys. Surveys will provide locations of the toe of slope around the perimeter of the reservoir and cross-sections at locations of storm pipes and slope stability issues, if any are identified. The base information will be supplemented with information from the original construction plans, where available. We propose to engage the service of S&ME, Inc. to provide geotechnical consultation. Representatives from the firm will visit the site and issue a report on its condition from a soils perspective and also make recommendations as to how to best remove woody vegetation and establish a grass cover. Should it be determined that a subsurface exploration be needed to supplement the design of repairs, we will negotiate these fees separately once the scope of work is known. After the plans and specifications are approved we will assist you during the bidding phase of the project. Our work will include preparing a bid schedule and bid documents, answering bidder questions, attending the pre-bid conference, attending the bid opening, tabulating the bids and making a recommendation as to the low responsible bidder. During the construction phase we will assist you with contractor requests for information, make periodic visits to the site to determine whether the work is being done according to the plans, issue reports of the results of such visits, make a final inspection to determine whether the work is satisfactorily completed, review and approve pay requests, and issue a Certificate of Completion. For purposes of this proposal we have assumed that we will visit the construction site two times per week over a three-month construction period. After all work is done we will make a required submittal on your behalf to the Safe Dams Office to transmit record drawings, approved modifications to the drawings, instrumentation locations, and our Certificate of Completion. D. Emergency Action Plan The plans and specifications for repair must also be accompanied by an Emergency Action Plan. An EAP is a written document which is used by the dam owner and emergency response personnel in the unlikely event that a potential emergency condition is identified as pending or in progress at the dam. It generally has six basic elements including: Notification Flowchart; Emergency Detection, Evaluation and Classification; Responsibilities; Preparedness; Inundation Maps; and Appendices. In this case, the EAP will be prepared in accordance with the requirements of the section on Emergency Action Plans given in the 2015 edition of Engineer Guidelines prepared and distributed by the Georgia Department of Natural Resources, Environmental Protection Division, Watershed Protection Branch, Safe Dams Programs. Perhaps the primary undertaking in order to prepare an EAP involves making a computer model to analyze what happens if a total breach of the dam occurs. This study uses cross-sections of the downstream drainageway taken from the Augusta GIS maps and simulates the development of a breach wave as it moves downhill from the dam. The water surface elevation of the wave is determined at each cross-section and a map of inundation is prepared from the data. Any structures falling inside the inundation zone are then identified as potentially at risk. As such, the occupants of such a structure need to be notified in the event of an emergency that could affect the dam. The address, telephone number and e-mail address, if available, are gathered for listing in the EAP. Because the reservoir has dam embankments on all four sides,we are assuming that we will have to develop at least six different breach models. 20 OF 29 REVISION DATE: June 2011 AUGUSTA r GE00.Gw The EAP will describe possible emergency scenarios, identify persons responsible for emergency response, describe the emergency response process, and allow documentation of emergency events including follow- up on how the EAP performed. The Notification flow chart will provide guidance and telephone numbers for emergency personnel. Finally, the Plan will document agreed upon EAP review and updating processes. Appendices will provide supplementary information such as dam design plans, inundation maps, flood information, and blank forms and logs. We also propose to meet with you and local emergency personnel to review the EAP. We will hold a tabletop test of the EAP once it is finalized. When complete we will provide 10 copies of the document along with a digital copy on disk. Additional copies can be supplied at our cost. E. Operation and Maintenance Manual We will prepare a single Operation and Maintenance(O&M)Manual for the two dams identified by the Safe Dams Office. We will utilize the template provided on the Safe Dams website. The work will include meetings with your operational and maintenance personnel to confirm ongoing policies and procedures so as to incorporate them into the manual. These services include making bi-annual visual inspections and reports of the condition of the dams for the year of 2019,preparing plans, specifications and design report for deficiencies noted and preparing supporting documentation including Emergency Action Plan(EAP) and Operation and Maintenance (O&M) Manual. This proposal outlines the project as we understand it,the scope of services offered, and the proposed fees. SCOPE OF WORK We understand that the Safe Dams Office of the Georgia Department of Natural Resources, Environmental Protection Division, notified you in 2017 that they have reclassified the reservoir from Category II, Low Hazard,to Category I, High Hazard. In doing this they also identified each cell of the reservoir as a separately numbered dam. These are now identified as Highland Avenue Water Treatment Plant Pond Dam#1 East and Highland Avenue Water Treatment Plant Pond Dam#2 West. Consequently, we will separately inspect and issue a report on each. It was decided that this would be monitored and then repaired during a AUGUSTA I CHARLESTON I HILTON HEAD Augusta Utilities Department March 25, 2019 Page 2 drier period of the year. Should we find that the height and storage capacity of the reservoir are both under Georgia Safe Dams threshold values, we will make a request on your behalf that the facilities be reclassified. B.
