This Amendment to the Health Risk Management Agreement(the"Amendment")is made and
entered into this 9th day of August 2018,by and between Healthstat,Inc.,a North Carolina Corporation
("HS")and Augusta Richmond County,Georgia,a political subdivision of the State of Georgia
("Employer").
WHEREAS, HS and Employer executed a Health Risk Management Agreement(the
"Agreement")dated January 1,2018 and have mutually agreed to amend the Agreement as set out
herein.
NOW,THEREFORE,in consideration of the premises and the mutual covenants contained
herein and in the Agreement, and for other good and valuable consideration,the receipt and adequacy of
which are hereby acknowledged,the parties agree as follows:
Effective July 25,2018:
• Exhibit"B": Service Cost is hereby amended to read as follows:
Clinic Staff Fees. .
Employer will no longer be staffmg the Clinic with a Medical Doctor. The Medical Doctor position will
be replaced with a Nurse Practitioner.
(Nurse Practitioner)Hourly Fee. Employer shall pay Healthstat an hourly rate to staff the Clinic with a
Nurse Practitioner.As of the Effective Date,the hourly rate due for this position is$103.00 per hour for
an estimated 40 hours per week.At the beginning of each annual anniversary date,this fee shall increase
by 4%.
The terms and conditions of the Agreement shall remain in full force and effect except as
expressly amended hereby. For purposes of clarity,the pre-existing Mid-Level Provider rates recited in
the Agreement for Augusta,GA remain unmodified by this Amendment.
IN WITNESS WHEREOF,the parties have executed this Amendment as of the day and year first above
written.
